摘要
目的探讨2型糖尿病肾病患者外周血胰岛素样生长因子-1(IGF-1)、转化生长因子-β(TGF-β)和血管内皮生长因子(VEGF)水平及临床意义。方法选取2014年10月—2016年5月收治的100例2型糖尿病肾病患者,根据24 h尿白蛋白排泄量分为正常白蛋白尿组34例、微白蛋白尿组32例、高白蛋白尿组34例,另选32例正常健康体检者作为对照组。采集研究对象外周血及尿样,对IGF-1、TGF-β和VEGF水平及其他临床指标进行检测分析。结果2型糖尿病肾病患者肌酐、尿素氮和收缩压随尿蛋白增高而增高(P<0.05)。高白蛋白尿组IGF-1水平高于对照组、正常白蛋白尿组和微白蛋白尿组(P<0.05);正常白蛋白尿组、微白蛋白尿组和高白蛋白尿组TGF-β和VEGF水平均高于对照组(P<0.01),高白蛋白尿组TGF-β水平高于正常白蛋白尿组和微白蛋白尿组(P<0.05),高白蛋白尿组和微白蛋白尿组VEGF水平高于正常白蛋白尿组(P<0.05)。结论检测IGF-1、TGF-β和VEGF水平能有效评估2型糖尿病肾病患者肾脏损伤程度,指导临床及时进行相关治疗。
Objective To investigate levels and clinical significances of insulin-like growth factor-1 (IGF-1) , transforming growth factor-β (TGF-β) and vascular endothelial growth factor (VEGF) in patients with type 2 diabetic nephropathy. Methods A total of 100 patients with type 2 diabetic nephropathy admitted during October 2014 and May 2016 were divided into normal albuminuria group ( n = 34), microalbuminuria group ( n = 32) and high albuminuria group (n = 34) according to urinary albumin excretion contents within 24 h, and other 32 healthy people were selected as control group. Peripheral blood and urine samples were collected, and IGF-1, TGF-β and VEGF levels and other clinical indexes were detected and analyzed. Results Creatinine, urea nitrogen and systolic blood pressure levels were increased with increasing urine protein level in patients with type 2 diabetic nephropathy (P 〈 0.05). IGF-1 level in high albumi- nuria group was significantly higher than those in control, normal albuminuria and microalbuminuria groups (P 〈 0.05 ). Levels of TGF-β and VEGF in normal albuminuria, microalbuminuria and high albuminuria groups were significantly higher than those in control group (P 〈 0.01 ) ; TGF-β level in high albuminuria group was significantly higher than that in normal albuminuria and microalbuminuria groups ( P 〈 0.05 ) ; VEGF levels in high albuminuria and microalbuminuria groups were significantly higher than those in normal albumiuuria group (P 〈 0. 05). Conclusion Detection for IGF-1, TGF-β and VEGF levels can effectively evaluate degree of renal damage and guide the clinical treatment in time in patients with type 2 diabetic nephropathy.
