摘要
目的探讨急性骨髓炎患儿血清中肿瘤坏死因子-α(TNF-α)、C-反应蛋白(CRP)及白介素-6(IL-6)水平动态变化及其临床意义。方法选取2014年3月-2015年6月托里县人民医院收治的急性骨髓炎患儿40例为研究对象,根据LEVIN评分标准将40例患儿分为优良预后组(27例)与不良预后组(13例),另选取先天性斜颈患儿32例为对照组,在手术前及手术后对急性骨髓炎患儿血清TNF-α、CRP及IL-6进行检测,同时检测对照组患儿血清TNF-α、CRP及IL-6水平进行对比分析。结果与对照组相比,优良预后组与不良预后组患儿的IL-6、CRP、TNF-α水平均明显升高,且不良预后组患儿的IL-6、CRP、TNF-α水平明显高于优良预后组与对照组,差异有统计学意义(P<0.05);与手术前对比,优良预后组患儿术后IL-6、CRP、TNF-α水平均明显降低,且低于术后不良预后组患儿IL-6、CRP、TNF-α水平,差异有统计学意义(P<0.05),不良预后组患儿IL-6、CRP、TNF-α水平手术前后无明显差异(P>0.05);经Pearson相关性分析显示,IL-6、CRP、TNF-α水平均与急性骨髓炎的发生与发展呈显著正相关(P<0.05)。结论 TNF-α、CRP及IL-6与急性骨髓炎发生与发展具有一定的相关性,可作为辅助诊断指标。
Objective To investigate the dynamic changes of serum tumor necrosis factor alpha （TNF-α） , C reactive protein （CRP） and interleukin-6 （IL-6） levels in children with acute osteomyelitis, and to analyze the clinical significance. Methods Selected 40 cases children with acute osteomyelitis from March 2014 to June 2015 in Toli County People’s Hospital as the objectives, according to LEVIN criteria, divided into good prognosis group （27 cases） and poor prognosis group （13 cases）, and another 32 cases children with congenital torticollis as control group, serum TNF-α, CRP and IL-6 were detected at admission and after surgery for them. Results Compared with the control group, children with good prognosis and poor prog-nosis group of theT NF-α, CRP and IL-6 levels were significantly elevated, and the TNF-α,CRP and IL-6 levels in the poor prognosis group were obviously higher than that in the good prognosis group and the control group, which the differences were statistically significant （P 〈0.05）; compared with the pre-sur-gery conditions, the TNF-α, CRP and IL-6 levelsin good prognosis group of post-surgery weresignificantly lower than that in poor prognosis group, which the differences were statistically significant （P 〈0.05）. But for the children in poor prognosis group, the TNF-α, CRP and IL-6 levels before and after surgery had no significant differences （P 〉 0.05 ） . The Pearson correlation analysis showed that significant-ly positive correlation of the TNF-α, CRP and IL-6 levels with the occurrence and development of the acute osteomyelitis （P 〈0.05）. Conclusion TNF-α, CRP and IL-6 levels are related with the development of a-cute osteomyelitis, which can be used as auxiliary diagnostic indexes.
