摘要
目的探讨既往有胆道手术史的胆总管结石患者再次行腹腔镜手术的临床效果。方法将我院收治的56例既往有开腹胆道手术史并需再行胆道手术治疗的患者作为研究对象,根据不同手术方式分为两组,各28例。观察组给予腹腔镜手术治疗,对照组给予常规开腹手术治疗。对比两组手术相关指标和并发症情况。结果观察组患者住院时间短于对照组(P<0.01),手术时间长于对照组(P<0.01),术后第1天疼痛VAS评分低于对照组(P<0.01);两组胃肠道功能恢复时间差异无统计学意义(P>0.05);观察组并发症发生率10.71%,低于对照组的35.71%(P<0.05)。结论既往有胆道手术史的胆道结石患者行腹腔镜手术虽具有一定难度,但通过合适病例选择和术前综合评估,可有效提高手术安全可行性。
Objective To investigate the clinical efficacy of laparoscopic surgery again in treating patients with common bile duct stones. Methods A total of 56 patients who had history of open biliary tract surgery and needed to be treated with biliary tract surgery again in our hospital were enrolled in the study. They were randomly divided into the observation group and the control group ,28 cases in each group. The observation group was treated with laparoscopic surgery for common bile duct stones, while the control group was treated with conventional open surgery for common bile duct stones. The surgery related indicators and complications were compared between the two groups. Results The length of hospital stay and pain VAS score on the 1 st day in the observation group after surgery were shorter and lower than those of the control group respectively （ P 〈 0. 01 ） but the surgical time of the observation group was longer than the control group （P 〈 0. 01 ）. The difference in the time of gastrointestinal function recovery between the two groups had no statistic significance （ P 〉 0. 05 ）. The incidence rate of complications was 10. 71% in the observation group,lower than 35.71% in the control group （ P 〈 0. 05 ）. Conclusion Laparoscopie surgery for patients with bile duct stones and the history of biliary tract surgery is difficult to operate but the safety and feasibility of the surgery can be improved through the selection of appropriate cases and comprehensive evaluation before surgery.
