射频消融与微波消融治疗肺肿瘤的对比研究 被引量:14

摘要 目的比较肺肿瘤射频消融与微波消融的近远期疗效及并发症,以期为临床选择消融手段提供依据。方法回顾性分析2007年3月至2014年12月间75例接受射频或者微波消融的肺肿瘤患者资料,比较近远期疗效及并发症。75例中射频消融43例,微波消融32例;男58例,女17例;原发性非小细胞肺癌51例,其他原发性肺肿瘤8例,转移性肺癌16例。两组患者治疗前的基线资料比较统计学处理无显著性差异(P>0.05)。结果近期疗效方面射频消融有效率(CR+PR)为79.1%(34/43),微波消融有效率为68.8%(22/32),两组资料经统计学处理无显著性差异(χ~2=1.033,P=0.309);射频消融和微波消融的0.5、1、2、3、5年生存率分别为95.0%、77.3%、55.3%、42.0%、33.6%和92.0%、75.0%、43.8%、39.8%、26.6%,两组资料经统计学处理无显著性差异(χ~2=0.203,P=0.653)。37例出现并发症,其中射频消融与微波消融分别发生21例和16例,两组资料经统计学处理没有显著性差异。结论射频消融与微波消融在肺肿瘤治疗方面均是安全的,在近远期疗效和安全性方面均无显著性差异,临床上均可以选择。 Objective To compare short-term and long-term efficacy and safety of radiofrequency ablation and microwave ablation in the treatment of lung tumors in order to guide the selecting ablation mode in clinical practice. Methods From March 2007 to December 2014, seventy-five patients who had been diagnosed of lung tumor underwent thermal ablation therapy , which included 43 cases of radiofrequency ablation and 32 cases of microwaves ablation. Short-term and longterm efficacy and complications were compared. There were 58 cases of male and 17 cases of female. Seventy-five patients included 51 cases of primary non-small cell lung cancer, 8 cases of other types of primary lung cancer and 16 cases of meta- static lung cancer. There were no significant differences in the baseline datum between radiofrequency ablation group and microwave ablation group. Results The effective rate( CR + PR) of radiofrequency ablation and microwave ablation was 79.1% (34/43) and 68.8% (22/32) respectively, there were no statistically significant differences between the two groups( χ2 = 1. 033 ,P = 0.309). The cumulative survival rates of 0.5,1,2,3,5 year in radiofrequency ablation group and microwave ablation group were 95.0% , 77.3% , 55.3% , 42.0%, 33.6% and 92.0%, 75.0%, 43.8%, 39.8% , 26.6% ,respectively. There were no significant differences (χ2 = 0. 203,P = 0. 653 ) in the survival rates between two groups. No patients died in the perioperative period. The complication rate was 49.3% (37/75) including 21 patients in radiofrequency ablation group (48.8% , 21/43 ) and 16 patients in microwave ablation group (50% , 16/32). There was no significant difference between the two groups (χ2 = 0. 010 ,P = 0. 921 ). Conclusion There were no significant differences in efficacy and safety between radiofrequency ablation and microwave ablation for the treatment of lung tumors. Both of them can be chosen clinically.
