摘要
目的:探讨中药洗剂在克罗恩病肛瘘治疗过程中的疗效。方法:回顾性分析2013年11月-2015年8月接受栓剂纳肛和/或药物灌肠治疗的38例克罗恩病肛瘘患者的临床资料。其中19例患者同时接受了中药洗剂治疗。比较2组患者在入院治疗后第4周、第8周的CDAI评分、PDAI评分的变化,并评价总体的疗效(治疗有效及治疗缓解)。结果:治疗第4周、第8周后,2组患者CDAI评分均明显降低,但2组之间无统计学差异(P>0.05)。洗剂组治疗第4周时的"分泌物"子项分(0.8±0.5 vs.1.4±0.7,P<0.01),第8周时的PDAI总分(3.1±1.9 vs.4.2±1.4,P<0.05)、"分泌物"子项分(0.1±0.3 vs.0.6±0.8,P<0.05)和"疼痛/活动"子项分(0.5±0.7 vs.1.1±0.6,P<0.05)均较对照组显著降低。在总体疗效方面,第8周时洗剂组的治疗有效率明显高于对照组(94.7%vs.63.2%,P<0.05)。结论:中药洗剂外用对克罗恩病肛瘘具有良好的疗效,其可以减少肛瘘局部的分泌液及减轻疼痛,促进肛瘘口的愈合。
Objective： To investigate the clinical efficacy of traditional Chinese medicine lotion in treatment of anal fistula in Crohn＇ s disease （CD）. Method： The clinical data of 38 CD patients with anal fistula who were treated with rectal suppository and/or enema were retrospectively retrived and analyzed. Among them 19 patients received treatment of traditional Chinese medicine lotion simultaneously. The changes of Crohn＇ s disease activity index （CDAI） and peri- anal disease activity index （PADI） at week 4 and week 8 after baseline were compared in two groups. The overall ef- ficacy was also evaluated （treatment response and treatment remission）. Results： CDAI scores decreased in both groups at week 4 and week 8, and no statistical difference existed between two groups （P〉0.05）. The sub-scores of ＂discharge＂ at week 4 （0.8 ± 0.5 vs. 1.4 ± 0.7, P〈0.01） and week 8 （0.1 ± 0.3 vs. 0.6 ± 0.8, P〈0.05）, ＂pain/restriction of activities＂ at week 8 （0.5 ± 0.7 vs. 1.1 ± 0.6, P〈0.05） and the PDAI at week 8 （3.1 ± 1.9 vs. 4.2 ± 1.4, P〈0.05） were significantly reduced in the lotion group in contrast with the control group. As for the overall efficacy, the rate of treatment response was comparatively higher in the lotion group at week 8 （94.7% vs. 63.2%, P〈0.05）. Conclusion： Traditional Chinese medicine lotion showed curative effects in treatment of anal fistula in CD. It reduced the secretions from fistulas, re- lieved the pain of the wound and promoted the closure of anal fistula.
