上消化道黏膜下肿瘤的超声内镜诊断与内镜下微创治疗价值研究 被引量:9

摘要 目的探讨超声内镜(EUS)对上消化道黏膜下肿瘤的诊断价值与内镜下治疗价值。方法对常规内镜发现的67例上消化道黏膜下肿瘤患者行超声内镜检查和CT检查,并经内镜下黏膜切除术(EMR)、内镜下黏膜剥离术(ESD)获取肿瘤标本,送病理组织学检查,比较EUS、CT与病理结果的诊断符合率。结果 67例病例中,对比消化道黏膜下肿瘤的诊断准确率,EUS(88.1%)明显高于CT(26.9%)(P<0.05)。EUS能清晰地显示出消化道管壁的层次,可较准确地判断出肿瘤的起源,但在显示恶性肿瘤侵犯周围组织及淋巴结转移方面,CT优于EUS。结论 EUS可较准确的判断上消化道黏膜下肿瘤的大小、起源及边界,并对其微创治疗的选择有重要的指导价值。 Objective To evaluate the diagnostic value of endoscopic ultrasonography (EUS) and endoscopic therapies under the guidance of EUS in upper gastrointestinal submucosal tumors. Methods Sixty-seven patients with protruding lesions of upper gastrointestinal submucosal tumors were examined by EUS and CT. And then the specimen was obtained through endoscopic mucosal resection (EMR) or endoscopic submucosal dissection (ESD) for the biopsy to compare with the diagnostic accordance rate of EUS and CT. Results The accuracy of EUS (88.1%) was significantly higher than that of CT (26.9%). EUS could clearly visualize the structure of the gastrointestinal wall, and accurately determine the origin of the tumor, but in the detection of infiltrating peripheral tissues and distant lymphnode metastasis,CT was superior to EUS. Conclusion EUS is an effective and reliable diagnostic method not only for accurately determining the size, origin and the location of upper gastrointestinal submueosal tumors but also for guiding minimally invasive therapy.
