摘要
目的为临床合理用药提供参考。方法对随机抽取的1441份出院病例/在院病例进行回顾性分析,总结不合理用药原因,针对反复出现的不合理用药问题进行重点解析。结果在抽取的1441份病历中,用药不合理病例223份,占病例总数的15.48%。不合理原因主要为溶媒选择不合理(27.27%)、要求冲管的未冲管(22.56%)、给药途径错误(21.21%)、药物静脉滴注浓度过大(11.77%)、药物选择不合理(7.4%)和重复用药(5.39%)等。结论本院住院医嘱中还存在很多不合理用药现象,临床药师通过医嘱点评可提高临床合理用药水平。
Objective To provide reference for the rational drug use in the clinical practice. Methods A total of 1441 inpatient/discharged medical records randomly selected were retrospectively studied. We summarized the irrational drug use and analyzed the recurring irrational drug use. Results Among the 1441 inpatient/discharged medical records, 223 showed unreasonable drug use, accounting for 15.48% of the total. The reason of irrational drug use mainly included inappropriate solvent selection（27.27%）, infusion tube not swilled as required（22.56%）, improper route of administration（21.21%）, too large drug concentration in intravenous drip（11.77%）, unreasonable drug selection（7.4%）, and repeated medication（5.39%）, etc. Conclusion Many irrational medical orders should be standardized in our hospital. Clinical pharmacists can help improve rational drug use through medical order review.
