摘要
目的:探讨营养风险筛查2002(NRS2002)在下肢骨折患者中的应用效果及护理方案。方法:选取本院骨科2015年6月~2016年6月收治的下肢骨折患者168例,术前应用NRS2002量表对患者进行评估,并根据评估结果将患者分为无营养风险组88例(NRS2002<3分,)及存在营养风险组80例(NRS2002≥3分),比较两组患者血清总蛋白(TP)、白蛋白(ALB)、血红蛋白(Hb)等水平差异,同时对营养风险组部分患者给予饮食支持干预,分析营养干预对营养风险患者营养指标及并发症的影响。结果:营养风险组患者血清TP,ALB,Hb水平均低于非营养风险组(P<0.05)。营养风险组中接受饮食支持的患者干预后血清TP,ALB,Hb水平高于非营养干预组(P<0.05),而术后贫血、骨折愈合不良、肺部感染、肌无力、肌肉萎缩等并发症发生率低于非营养支持组(P<0.05)。结论:NRS2002在下肢骨折营养不良高危人群筛选中具有一定的应用价值,通过对下肢骨折营养不良风险人群进行饮食干预可有效改善患者营养状况,降低患者术后并发症发生率。
Objective To discuss the application effect and nursing plan of nutritional risk screening 2002 (NRS2002) in the patients with fracture of lowerlimbs. Methods: Selected 168 patients with fracture of lower limbs admitted in orthopedics department of our hospital from June 2015 to June 2016, and be-fore operation, NRS2002 scale was used to evaluate the patients, and the patients were divided into 88 patients in the group without nutrient risk ( NRS2002〈3 scores) and 80 patients with nutrient risk (NRS2002 〉3 scores) ; patients in the two groups were compared in the difference of total serum protein(TP), albumin (ALB), hemoglobin (Hb) and other levels, and diet support interference was given to some patients in the nutrient risk group to analyzethe influence of nutrient interference on the nutrient indicators and complications of patients in the nutrient risk group. Results : The serum TP, ALB and Hblevels of patients in the nutrient risk group were lower than those of patients in the non - nutrient risk group on average ( P 〈 0.04). For the patients receiv-ing diet support in the nutrient risk group, the serum TP, ALB and Hb levels after interference were higher than those of patients in the non - nutrient inter-ference group (P 〈 0.05 ), while the post - operation complications like anemia, poor healing of fracture, pulmonary infection, myasthenia and muscle at-rophy of patients in the nutrient risk group were lower than those of patients in the non - nutrient support group ( P 〈 0.05 ). Conclusion : NRS2002 had cer-tain application value in the screening of high risk group with poor nutrition of fracture of lower limbs, and reasonable diet interference for the group withpoor nutrition risk of fracture of lower limbs could effectively improve the patients' nutrition and reduce the incidence of post - operation complications.
