摘要
目的探讨气道压力释放通气治疗急性呼吸窘迫综合征的临床效果。方法 76例急性呼吸窘迫综合征患者,随机分为对照组和观察组,各38例。对照组在常规治疗基础上联合呼气末正压通气治疗,观察组在常规治疗基础上联合气道压力释放通气治疗。比较两组患者血氧饱和度(SpO_2)、氧分压(PO_2)、二氧化碳分压(PCO_2)、呼吸指数(RI)、氧合指数(OI)变化及肺部X线片情况。结果两组患者SpO_2、PO_2水平比较差异无统计学意义(P>0.05);观察组患者PCO_2、RI、OI水平分别为(4.05±0.6)mm Hg(1mm Hg=0.133 k Pa)、(1.68±0.41)、(38.42±4.37),优于对照组的(5.03±0.75)mm Hg、(1.98±0.49)、(32.91±4.87),差异具有统计学意义(P<0.05)。观察组患者病情恶化率为18.42%,低于对照组的39.47%,差异具有统计学意义(P<0.05)。结论对急性呼吸窘迫综合征患者实施气道压力释放通气联合常规治疗,可改善患者血气分析指标及心肺功能,抑制病情发展,值得临床推广应用。
Objective To explore the clinical efficacy of airway pressure release ventilation in the treatment of acute respiratory distress syndrome. Methods A total of 76 acute respiratory distress syndrome patients were randomly divided into control group and observation group, with 38 cases in each group. The control group received positive end expiratory pressure treatment on the basis of conventional therapy, and the observation group received airway pressure release ventilation on the basis of conventional therapy. Comparison were made on blood oxygen saturation(SpO2), partial oxygen pressure(PO2), partial pressure of carbon dioxide(PCO2), respiratory index(RI), oxygenation index(OI) changes and lung X-ray slice in two groups. Results There were no statistically significant difference in Sp O2 and PO2 level in two groups(P〉0.05). The observation group had PCO2, RI and OI level respectively as(4.05±0.6) mm Hg(1mm Hg=0.133 k Pa),(1.68±0.41) and(38.42±4.37), which were better than(5.03±0.75) mm Hg,(1.98±0.49) and(32.91±4.87) in the control group. Their difference had statistical significance(P〈0.05). The observation group had deterioration rate as 18.42%, which was lower than 39.47% in the control group, and the difference had statistical significance(P〈0.05). Conclusion Implement of airway pressure release ventilation combined with conventional therapy can improve blood gas analysis index and cardiopulmonary function in patients with acute respiratory distress syndrome, inhibit the progression of disease. So it is worthy of clinical popularization and application.
