摘要
目的研究高压氧(hyperbaric oxygen,HBO)联合针灸预处理对急性心肌缺血再灌注损伤模型大鼠的保护效应。方法将36只SD大鼠按数字表法随机分为3组:假手术组、模型组、HBO联合针灸组(联合组),每组各12只。联合组针刺大鼠双侧内关穴后维持留针状态,放入氧舱内,再于0.2MPa下暴露1h,每天2次,连续暴露5d后,建立心肌缺血再灌注模型。模型组和联合组均采用结扎大鼠冠状动脉左前降支40min,再灌注2h的方法,建立心肌缺血再灌注模型;假手术组仅行开胸手术而不造模。再灌注结束后检测3组大鼠左心室收缩压(left ventricularsy stolic pressure,LVSP)、左心室舒张压(left ventricular diastolic pressure,LVDP)、左心室内压最大上升和下降速率[±(dP/dt)max]等心功能指标;HE染色观察心肌损伤程度;TUNEL法检测心肌缺血区心肌细胞凋亡率。结果模型组大鼠心功能指标LVSP为(112.64±8.49)mmHg、LVDP为(6.75±1.34)mmHg、+(dP/dt)max为(2970.15±137.91)mmHg/s、-(dWdt)max为(2936.86±179.50)mmHg/s,而联合组上述指标分别为(123.33±7.86)mmHg、(6.13±1.56)mmHg、(3046.56±137.91)mmHg/s、(3022.30±283.73)mmHg/s;模型组大鼠心肌细胞平均凋亡率为(94.58±34.01)%,而联合组为(42.17±27.61)%。2组上述指标比较差异均有统计学意义(P〈0.05)。与模型组相比,联合组心肌缺血再灌注损伤显著减轻,心肌组织病理性损伤减轻,心功能改善,心肌细胞凋亡率减轻。结论高压氧联合针刺内关穴后静留针预处理对心肌缺血再灌注损伤模型大鼠有显著的保护效应。
Objective To investigate the protective effects of hyperbaric oxygen (HBO)and acupuncture preconditioning on the injury induced by acute myocardial ischemia reperfusion in rat model. Methods Thirty-six SD rats were randomly divided into 3 groups: the sham surgery group (or the sham group) , the model group and the HBO and acupuncture group (or the combined group), each consisting of 12 animals. The combined group received acupuncture treatment at Neikuan acupoint bilaterally with the needles remaining, then was placed into the animal chamber and exposed to HBO at a pressure of 0.2 MPa for 1 hour a session, 2 sessions a day, and for a succession of 5 days. Then, the myocardial ischemia reperfusion injury model was established. For the model and the combined groups, the myocardial ischemia reperfusion injury model was established by ligation of the left anterior descending branch of the coronary artery for 40 minutes and then reperfusion was performed for 2 hours. The sham group only received open chest surgery without the development of the model. Cardiac functions, such as left ventricular systolic pressure (LVSP), left ventricular diastolic pressure (LVDP), maximal increasing and decreasing rates of left intraventricular pressure [ ± (dP/dt)max] were monitored in the rats of the 3 groups after reperfusion. Morphological injury was measured with Hematoxylin and Eosin (HE) staining, and apoptosis of cardiac cells in the affected area was detected with TUNEL. Results LVSP of the model group is ( 112.64 ± 8.49) mmHg, LVDP is (6.75 ± 1.34) mmHg, + (dP/dt) max is (2 970.15 ± 137.91 ) mmHg/s, - (dp/dt) max is (2 936.86 ± 179.50) mmHg/s; LVSP of the combined group is (123.33 ±7.86) mmHg, LVDP is (6.13 ± 1.56) mmHg, + (dp/dt) max is (3 046.56± 137.91 ) mmHg/s, - (dp/dt) max is ( 3 022.30 ± 283.73 ) mmHg/s. The apoptosis rate of the model group is (94.58 ± 34.01 )%, and the apoptosis rate of the combined group is (42.17 ± 27.61 )%. As compared with that of the model group, reperfusion injury was significantly less severer for the combined group, and morphological lesion in the affected area was alleviated, cardiac function improved and apoptosis of cardiac cells was also alleviated considerably. Conclusions HBO combined with acupuncture at Neikuan acupoint could produce good protective effect on the rat with myocardial ischemia reperfusion injury.
