摘要
目的探讨人尾加压素Ⅱ对脑出血病情、预后判断以及治疗方案选择的意义。方法选取2015年3月~2016年3月在我院住院的120例脑出血患者,并设置30例健康体检者为对照组。120例患者按病情严重程度、治疗方案、预后进行不同的分组。检测所有入组患者入院时和入院后第3、5、7天血浆中人尾加压素Ⅱ水平。结果入院时脑出血组患者人尾加压素Ⅱ水平显著高于健康对照组（P〈0.05）。人尾加压素Ⅱ水平与病情的严重程度呈正相关,三组间人尾加压素Ⅱ水平差异有统计学意义（P〈0.05）。预后良好组第7天人尾加压素Ⅱ水平接近健康对照组（P〉0.05）,预后不良组发病后7 d内血浆人尾加压素Ⅱ水平持续高于预后良好组（P〈0.05）。当人尾加压素Ⅱ水平≥12.56 ng/m L时,外科手术治疗的预后显著高于药物保守治疗（P〈0.05）;当人尾加压素Ⅱ水平〈12.56 ng/m L时,外科手术治疗的预后与药物保守治疗接近（P〉0.05）。结论人尾加压素Ⅱ水平可以作为脑出血患者病情及预后判断的指标,并可以指导脑出血患者治疗方案的选择。
Objective To explore the effect of human urotensin Ⅱ on the pathological condition and prognosis of cerebral hemorrhage and on the treatment options. Methods A total of 120 patients with cerebral hemorrhage who were in our hospital from March 2015 to March 2016 were selected, and 30 healthy subjects were selected as the control group.The 120 patients were divided into groups according to the severity of the disease and the difference in treatment and prognosis.Plasma levels of human urotensin II of all patients enrolled were measured at admission,on days 3, 5, and 7after admission. Results On admission,the level of urotensin Ⅱ in patients with cerebral hemorrhage was significantly higher than that of Healthy control group（P0.05）. The level of human urotensin Ⅱ was positively correlated with the severity of the disease, and there was significant difference between the three groups（P0.05）. There were similar levels of plasma urotensin Ⅱ between the good prognosis group and the healthy control group on the seventh day（P0.05）, and the level of plasma urotensin Ⅱ in the poor prognosis group was higher than that in the good prognosis group within 7days（P〈0.05）. When the level of human urotensin Ⅱ≥12.56 ng/m L, the prognosis of surgical treatment was significantly better than the conservative treatment（P〈0.05）; when the level of urotensin Ⅱ12.56 ng/m L, the prognosis of surgical treatment and drug conservative treatment was close（P〉0.05）. Conclusion The level of human urotensin Ⅱ can be used as an indicator of the pathological condition and prognosis of patients with cerebral hemorrhage, and can guide the treatment options for cerebral hemorrhage patients.
