摘要
[目的]探讨人工全髋关节置换术后股骨偏心距与聚乙烯磨损的相关性。[方法]用计算机辅助测量方法测量髋关节置换聚乙烯内衬的线性磨损。以股骨头中心参照于髋臼杯开口面轴线的位移,计算线性磨损的大小与方向。对2008年11月~2015年10月本院骨科78例使用非骨水泥型金属对聚乙烯假体患者的X线片进行股骨偏心距和聚乙烯磨损的测量,平均随访时间4.08年。[结果]平均线性磨损率(0.18±0.04)mm/年,平均线性磨损方向与髋臼杯轴线的夹角(33.0±18.2)°。术中股骨偏心距得到重建组的线性磨损率小于未得到重建组(P<0.001)。股骨偏心距得到重建组与未得到重建组的磨损方向差异无统计学意义(P=0.210)。对股骨双侧偏心距差值与年磨损率进行相关分析,两者成正相关性(r=0.669,P<0.001)。[结论]人工全髋关节置换术中重建股骨偏心距可减慢聚乙烯内衬磨损速度,延长髋关节假体的寿命,改善远期效果。
[Objective ] To explore the correlation between femoral offset and polyethylene wear after total hip arthroplasty (THA) . [Methods] A computer-assisted method was applied to measure the linear polyethylene wear after total hip arthroplasty. The degree and direction of linear wear were determined by measuring the movement of the femoral head center relative to the aeetabular cup axis. The radiographs of 78 cementless THA patients admitted to our hospital from November 2008 to October 2015 were used to examine their femoral offset and polyethylene wear. The mean follow-up time for these patients was 4.08 years. [Results] The average linear wear rate was 0.18±0.04 mm per year, and the angle between the average linear wear direction and the aeetabular cup axis was 33.0°±18.2°. The liner polyethylene wear rate of the femoral offset reconstruction group was significantly lower than that of the group without femoral offset reconstruction (P 〈0.001). There was oo significant difference in polyethylene wear direction between the two groups (P = 0.210). The bilateral femoral offset difference was positively correlated with the polyethylene wear per year (r = 0.669, P 〈0.001). [Conclusions] Reconstruction of the femoral offset in THA can reduce the speed of polyethylene wear, increase the survival time of prosthesis, and improve the long- term efficacy.
