摘要
目的调查抗菌药物专项整治活动前后医院抗菌药物使用情况、存在问题及临床药师干预不合理用药情况和干预采纳情况，探讨临床药师在促进合理使用抗菌药物方面尚需改进之处。方法运用成都市新都区中医医院信息系统与临床药学管理系统，统计、对比医院2010（整治前）、2011年（整治前期）及2013年（整治后期）各项抗菌药物临床使用数据情况；统计、分析2011～2013年整治活动期间，抗菌药物不合理使用存在问题及临床药师干预采纳情况。结果在整治前、整治前期、整治后期，医院抗菌药物使用数据逐渐改善，其中，门诊抗菌药物使用率分别为36．7％、28．6％、17．5％；住院抗菌药物使用率分别为73．1％、66．3％、54．8％；住院抗菌药物使用强度分别为67．0、58．6、39．0。临床药师提出的干预用药病例建议，74。9％被医生采纳。80．0％会诊病例转归为治愈或好转。结论临床药师在促进临床抗菌药物合理使用方面起到积极作用，但仍应加强业务学习，增强与医护的交流，进一步提高医生对临床药师干预采纳率。
Objective To explore the improvement of clinical pharmacists in promoting rational use of antibiotics by in- vestigating the use of antibiotics and the existing problems in hospital and the situation of clinical pharmacists interven- tion in irrational antibiotics use and intervention adoption before and after the special rectification activity. Methods By using the information system and clinical pharmacy management system of Xindu Hospital of Traditional Chinese Medicine of Chengdu City, clinical data of antibiotics in 2010 （before the special rectification activity）, 2011 （early pe- riod of activity） and 2013 （later period of activity） were studied; the problem of irrational use of antibiotics and clinical acceptance of clinical pharmacist intervention from 2011 to 2013 （during the activity） were analysed. Results At three stages of activity, the data of antibiotics using were gradually rational. Among them, outpatient use rate of antibiotics were 36.7%, 28.6%, 17.5% respectively; inpatient use rate of antibiotics were 73.1%, 66.3%, 54.8% respectively; in- patient AUD were 67.0, 58.6, 39.0 respectively 74.9% of respectively. Clinical pharmacist intervention antibiotics rec- ommendations of were adopted by the doctors. 80.0% of consultation cases were cured or improved. Conclusion Clini- cal pharmacists play a positive role to promote clinical rational use of antibiotics, but still should strengthen the busi- ness study, enhance the communication with doctors and nurse, further improve the acceptance of clinical pharmacist intervention by doctors.
