天津地区45229名健康体检者男女性尿酸水平差别及高尿酸血症影响因素 被引量:28

The uric acid level difference between males and females and the influencing factor of hyperuricemia in 45229 residents with the physical examination of Tianjin area

摘要 目的回顾性分析天津地区健康体检人群男女性尿酸(UA)水平变化趋势及影响因素,为个体化健康指导意见提供理论依据。方法选择2014年在天津市肿瘤医院预防体检中心体检并符合标准的45 229名健康体检者为研究对象,检测血清UA、甘油三酯(TG)、总胆固醇(TC)、空腹血糖(FPG),用SPSS 19.0统计软件包进行t检验、多元线性回归和多因素logistic回归分析,趋势判断采用Cox-Staut趋势存在性检验。结果男性UA水平随着年龄的增加而降低,女性UA水平随着年龄的增加而升高,差异均有统计学意义(P<0.01)。多元线性回归分析显示,男性UA与TC、TG呈正相关,与年龄和FPG呈负相关(r值分别为0.100、0.269、-0.139和-0.110,P<0.01);女性UA与TC、TG和年龄均呈正相关(r值分别为0.208、0.346和0.191,P<0.01)。不同性别、年龄HUA检出率与UA水平变化趋势一致,男性HUA检出率从40岁开始出现显著降低,而高FPG检出率则显著升高。logistic多因素回归分析显示,TC>5.71 mmol/L、TG>1.81 mmol/L为男性HUA的危险因素(OR值分别为1.288、2.553),FPG≥5.87 mmol/L为男性HUA的保护因素(OR=0.766);TC>5.71 mmol/L、TG>1.53 mmol/L、FPG≥5.53 mmol/L均是女性HUA的危险因素(OR值分别为1.347、3.969和1.659)。结论年轻男性以及≥50岁女性应积极关注UA水平,高UA水平与血脂异常密切相关,男性UA水平及HUA检出率随年龄的变化趋势与FPG有关。 Objective To retrospective analysis the levels, change trend and influencing factors of the serum uric acid(UA) in male and female residents of Tianjin area, and to provide the basis for treating the hyperuricemia(HUA). Methods In 2014, 45 229 healthy subjects were selected from the residents performing the physical examination in Tianjin area. The serum uric acid(UA),triglyceride(TG), total cholesterol(TC) and fasting plasma glucose(FPG) were determined, and the data were analyzed by t test,multiple linear regression and logistic regression. The trend was assessed with Cox-staut test. Results The male UA level reduced with age, but the female UA level enhanced with age(P〈0.01). The multiple linear regression showed that male UA level correlated positively with TC and TG, and male UA level correlated negatively with age and FPG(r values were 0.100, 0.269,-0.139 and-0.110,respectively, P〈0.01); the female UA level correlated positively with TC, TG and age(r values were 0.208, 0.346 and 0.191,respectively, P〈0.01). The HUA detection rates in different gender and age groups were similar to the change trends of UA, the male HUA detection rate decreased significantly but high FPG detection rate enhanced significantly from 40 years old. The multivariate logistic regression analysis showed that TC5.71 mmol/L and TG1.81 mmol/L were the risk factors of male HUA(OR values were 1.288 and 2.553); FPG≥5.87 mmol/L was the protection factor of male HUA(OR=0.766); TC5.71 mmol/L, TG1.53mmol/L and FPG ≥5.53 mmol/L were the risk factors of female HUA(OR values were 1.347, 3.969 and 1.659, respectively).Conclusion Young male and females(50 ≥ years old) should pay attention to UA level, high UA level is related closely to dyslipidemia, the change trends of male UA level and HUA detection rate are related with FPG.
