妊娠期高血压疾病与甲状腺功能异常的相关性分析 被引量:6

摘要 目的探讨妊娠期高血压疾病与甲状腺功能异常的相关性。方法选择2010年6月至2015年6月本院收治的146例妊娠期高血压疾病孕妇为研究对象,根据妊娠期高血压疾病种类将其分为妊娠期高血压组(72例)、轻度子痫前期组(39例)和重度子痫前期组(35例)。选取本院同期健康孕妇200例纳入对照组。分别于孕早期(妊娠第12周)、孕中期(妊娠第24周)及孕晚期(妊娠第36周)3个时间点检测并比较各组孕妇游离甲状腺素(free thyroxine,FT_4)和促甲状腺素(thyroid stimulating hormone,TSH)水平。结果孕早期,四组孕妇FT_4和TSH水平比较无显著差异(P>0.05)。孕中期,重度子痫前期组孕妇FT_4水平明显低于妊娠期高血压组、轻度子痫前期组及对照组(P<0.05),妊娠期高血压组和轻度子痫前期组孕妇FT_4水平组间比较无明显差异(P>0.05)。孕中期,重度子痫前期组孕妇TSH水平明显高于妊娠期高血压组、轻度子痫前期组及对照组(P<0.05),妊娠期高血压组和轻度子痫前期组孕妇TSH水平组间比较无明显差异(P>0.05)。孕晚期,重度子痫前期组孕妇FT_4水平低于孕中期(P<0.05),TSH水平高于孕中期(P<0.05)。结论妊娠期高血压疾病孕妇妊娠期间TSH水平较高,FT_4水平较低。妊娠期间甲状腺功能减退引起的血脂紊乱和心血管系统的变化可能参与妊娠期高血压疾病的发生。 Objective To study the correlation between hypertensive disorders of pregnancy and thyroid dysfunction. Method 146 pregnant women with hypertensive disorders of pregnancy were selected as objects of study, according to the types of gestational hypertension, they were divided into gestational hypertension group (72 cases), mild preeclampsia group (39 cases) and severe preeclampsia group (35 cases). A total of 200 healthy pregnant women at the same period in our hospital were selected into control group. The free thyroxine (FT4) and serum thyroid stimulating hormone (TSH) for all the pregnant women above in the early stages of pregnancy (12 weeks of gestation), mid pregnancy (24 weeks of gestation) and late pregnancy (36 weeks of gestation) were detected and compared. Result In the early pregnancy, there were no significant differences of FT4 and TSH among all grouPs (P 〉 0.05). In the pregnant metaphase, FT4 level of severe preeclampsia group was lower than gestational hypertension group, mild preeclampsia group and control group (P 〈 0.05). There was no significant difference of FT4 between gestational hypertension group and mild preeclampsia group in the mid pregnancy (P 〉 0.05). In the pregnant metaphase, TSH level of severe preeclampsia group was higher than gestational hypertension group, mild preeclampsia group and control group (P 〈 0.05). There was no significant difference of TSH between gestational hypertension group and mild preeclampsia group in the mid pregnancy (P 〉 0.05). Compared with pregnant metaphase, severe preeclampsia group in the late pregnancy had lower FT4 level and higher TSH level (P 〈 0.05). Conclusion Pregnant women with hypertensive disorders of pregnancy show higher TSH level and lower FT4 level. Hypothyroidism during pregnancy may be involved in the happening of hypertensive disorders of pregnancy.
