摘要
目的探讨血浆脂蛋白相关磷脂酶A2(Lp—PLA2)水平与急性心肌梗死(AMI)的相关性。方法应用ELISA法检测85例AMI患者和50例稳定型冠心病患者血浆Lp—PLA2水IF,分析Lp-PLA2水平与肌酸激酶同工酶(CK—MB)、肌红蛋白和肌钙蛋白I(cTnl)等的相关忭,行进行受试者工作特征(ROC)曲线分析。结果(1)两组患者血脂水平和同型半胱氨酸(Hcy)比较差异有统计学意义(P〈0.05);AMI组CK-MB、肌红蛋白和cTnl的表达较稳定型冠心病组明渺升高(P〈0.05);(2)AMI患者血浆Lp—PLA2水平明显高于稳定型冠心病患者(P〈0.05),血浆Lp—PIJA2水平与CK—MB、肌红蛋白和cTnI呈正相关(P〈0.05);ROC曲线分析发现血浆Lp-PLA2的ROC曲线下面积为0.88,比CK—MB(0.83)和肌红蛋白(0.86)均高,但低于cTnI(0.94);(3)AMI患者PCI术后3天LP—PLA2的表达较术前和术后1天明显降低(P〈0.05);(4)AMI患者中三支血管病变耆血浆Lp—PLA2的表达较双支病变和单支病变明显升高(P〈0.05),双支病变者较毕。支病变者也明显升高(P〈0.05)。结论血浆Lp—PLA2水平可能与AMI发病有关,可作为一个判断AMI患者心肌损伤严重程度的新型标志物。
Objective To investigate the relationship of lipoprotein-associated phospholipase A2 (Lp-PLA2) and acute myocardial infarction(AMI). Methods 85 AMI patients were recruited into AMI group and 50 patients with stable chronic coronary artery disease (SCAD) were recruited into SCAD group. We detected plasma Lp-PLA2 levels by enzyme-linked immunosnrbent assay (EI,ISA), and analyzed its correlation with CK-MB, myoglobin and cTnI as well as ROC curve analysis. Results ( 1 ) There were no statistical differences in age, gender and basic history in two groups ( P 〉 0.05 ) ; and there were differences in lipid and homocysteine (Hcy) levels ( P 〈 0.05 ), while CK-MB, myoglobin and cTnI levels in AMI significantly increased (P 〈 0. 05 ). (2) Plasma Lp-PLA2 in AM1 cases were signifieantly higher( P 〈 0.05 ), and were correlated with CK-MB, myoglobin and eTnl ( P 〈 0. 05 ). ROC curve analysis found that plasma Lp-PLA2 AUC was 0.88, CK-MB was 0.83, myogluhin was 0.86 and cTnI was 0.94. (3)After PCI, plasma Lp-PLA2 were much lower in the third day than those in preoperative and postoperative 1 day(P 〈0. 05 ). (4)The expression of Lp-PLA2 in 3 vascular lesions in A MI cases increasedsignificantly than two vascular lesions and single vascular lesion ( P 〈 0.05 ) , and Lp-PLA2 expression of two vascular lesions cases were significant higher than single vascular lesion( P 〈 0.05 ). Conclusion Plasma Lp-PLA2 level may be associated with AMI and it can be used as a new biomarker for AMI, which may be used to judge the severity of myocardial injury.
