摘要
目的了解医院临床分离病原菌的分布及其耐药特征,为临床合理使用抗菌药物提供依据。方法采用细菌分离鉴定技术和药敏试验方法,对某医院住院患者送检标本分离的病原菌种类和药敏试验结果进行调查分析。结果该医院2014年1月-6月在1 196例标本中,共分离出293株病原菌,阳性检出率为24.50%,其中革兰阴性菌124株,革兰阳性菌77株,真菌92株,以大肠埃希菌、铜绿假单胞菌、表皮葡萄球菌、肺炎克雷伯菌和金黄色葡萄球菌为优势菌群。大肠埃希菌对阿莫西林、头孢噻吩、阿莫西林/棒酸、SMZ、头孢呋辛的耐药性较高,其耐药性>60%;铜绿假单胞菌对氨苄西林/舒巴坦、SMZ、替卡西林/棒酸耐药性高达90%以上;肺炎克雷伯菌对阿莫西林、头孢噻吩、头孢呋辛、SMZ、氧哌嗪有较高的耐药性。结论临床医生应重视病原菌的耐药性监测,了解细菌耐药性的变迁,合理选择和使用抗菌药物。
Objective To investigate the distributions and antimicrobial resistances of the bacteria in hospital to provide the basis for clinical rational use of antimicrobial agents. Methods Bacteria isolated identification technologies and susceptibility testing methods were used to investigate and analyze the species of pathogen and antimicrobial susceptibility test results submission of samples in a hospital inpatients. Results 293 strains of pathogenic bacteria were isolated from the 1 196 cases of specimens from hospitals in the first half of 2014,with the positive rate of 24. 50%,among which the gram negative bacteria were 124 strains,the gram positive bacteria were 77 strains,the fungus were 92 strains. E. Coli,Pseudomonas aeruginosa,epidermis staphylococcus,Klebsiella pneumoniae and Staphylococcus aureus were the dominant flora. The cefuroxime resistance of Escherichia coli to amoxicillin,cephalothin,amoxicillin/clavulanic acid,SMZ was high,their resistances were greater than 60%; the cefuroxime resistance of Pseudomonas aeruginosa to Ampicillin/sulbactam,SMZ,ticarcillin/clavulanic acid were up to 90%;the cefuroxime resistance of Klebsiella pneumoniae to amoxicillin ,cephalothin ,cefuroxime ,SMZ ,piperacillin were also high .Conclusion Clinical doctors should pay attention to monitor the drug resistance of pathogenic bacteria,to understand the changes of bacterial drug resistance,rational selection and use of antimicrobial agents.
