摘要
仲景以桂枝茯苓丸治疗妊娠病,故炼蜜和丸,白蜜润肠补中,丸者缓也,药量逐渐增加,一则固胎扶正,二则有形之物,当以缓消。全方时刻顾护正气与阴血,免伤胎气,通因通用治漏下,活血通脉,瘀水同消,祛邪不伤正。胶艾汤以胶艾与四物配伍,意在求本,组方用药不重炭黑炒末之品塞流,而是通过充养血海、调和冲任、温固经脉以和气血,血循常道、瘀血归经则藏泄有度,下漏自止。当归芍药散肝脾同调,气血兼顾,补泻并用,血水共治,仲景强调肝与血对女性的重要性在此略见一斑。散者散也,用散剂治疗血水互结达到标本同治。半夏厚朴汤行气解郁,化痰除涎,辛开苦降,标本同治,用药紧扣病机,不仅治疗梅核气,还能治疗各种痰凝气滞导致的精神病,胃脘病,胸肺疾病等。甘麦大枣汤补益心脾,生金荣木,心肺肝脾均得以滋养,烦躁自除。温经汤用药覆盖寒、热、虚、瘀四大病机,正是"天地温和,则经水安静也"。虽病症错杂,从配伍来看仲景治疗妇人出血证思路层次清晰。仲景治疗妇人病无不以肝和血为中心,强调女子以血为本,以肝为先天。然治肝不忘脾,治血不离水,是"见肝实脾""血不利则为水"思想的贯穿。仲景用药精简但紧扣病证,多种药味均可相互结合成有意义的药对,补泻并用、寒热相合、动静搭配等特点突出了仲景用药的灵活性。各种治法也无不体现了审证求因,审因论治,治病求本的原则。
