摘要
目的通过对县、乡、村三级医疗机构卫生服务质量间的相互影响情况进行分析,为后期门诊和住院服务质量相关政策的调整及干预措施的实施提供基础依据。方法选用结构方程模型对宁夏回族自治区2012年"创新支付制度,提高卫生效益"项目资料进行分析。结果县、乡、村三级医疗机构之间的门诊/住院服务质量之间的相互影响均为正向且有意义(P<0.05),其中在扣除了同一级别医疗机构之间的门诊和住院服务质量的相互影响外,乡村两级的相互影响程度最高。结论不同医疗机构间的门诊、住院服务质量存在相互影响的情况,因此在后续干预措施的制定和实施过程中应当考虑到这种相互作用,以便更好地为居民的卫生保健进行服务。
Objective To provide basic data for policy adjustment related outpatient and inpatient service quality and interventions through investigating of mutual influences of outpatient and hospital service quality in three-tier medical institutions of county, township and village. Methods The data from the project ＂innovation of payment system, improvement of health efficiency＂ in Ningxia in 2012 was analyzed by structural equation model. Results Mutual influences of outpatient and hospital service quality in three-tier medical institutions of county, township and village was positive and meaningful （P〈0.05）. Mutual influences between ownship and village were highest, when mutual influences in the same level medical institution were excluded. Conclusion Outpatient and hospital service quality in different level medical institutions Influenced each other. Therefore we should consider this interaction when subsequent interventions were established and carried out to better sevice for residents＇ health care.
