摘要
目的研究超声弹性成像联合正电子发射断层显像（PET）加X线计算机体层成像（CT）对甲状腺癌的诊断价值。方法选取2014年8月～2015年8月于我院确诊的76例甲状腺癌患者，其中共有93个甲状腺结节，所有患者术前行超声弹性成像（UE）和PET／CT检测，且所有结节均于术后获得病理证实，比较分析UE及UE联合PET／CT的诊断结果．并对两种诊断方式的敏感度、特异度、准确度进行比较。结果在93个甲状腺结节中，其中39个为恶性结节，54个为良性结节；采用UE＋PET／CT联合方式诊断的特异度、准确度均比uE方式要高[94．44％比81．48％，94．62％比84．95％]，差异有统计学意义（P〈0．05）。结论UE和PET／CT在甲状腺肿瘤的良恶性诊断中均存在较高价值，两种诊断方法联合使用后，能有效提高甲状腺癌的诊断准确性，对明确治疗方案具有积极意义，值得临床推广。
Objective To study the value of ultrasound elastography imaging combined with positron emission tomography （PET）/X-ray computed tomography （CT） in the diagnosis of thyroid carcinoma.Methods 76 cases patients with thyroid cancer diagnosed in our hospital from August 2014 to August 2015 had 93 thyroid nodules.All patients were detected by ultrasonic elastography （UE） and PET/CT before operation,all nodules were verified by pathology after operation.Diagnostic results of UE and UE combined with PET/CT,and sensitivity,specificity and accuracy of the two diagnostic methods were compared.Results 93 thyroid nodules,in which had 39 malignant nodules and 54 benign nodules.The specificity and accuracy for UE ＋PET/CT joint diagnosis were higher than UE diagnosis [94.44% vs 81.48%,% 94.62 vs 84.95%],with a significant difference （P〈0.05）.Conclusion UE and PET/CT in the diagnosis of benign and malignant thyroid tumor have a higher diagnosis value,two diagnostic methods used in combination,can effectively improve the diagnosis accuracy of thyroid cancer, have positive significance for the explicit treatment and worth of clinical promotion.
