摘要
目的探讨外源性硫化氢对静脉移植物内膜增生的影响。方法建立大鼠颈外静脉移植模型,24只雄性大鼠随机分成对照组和实验组,每组12只,实验组给腹腔注射硫化氢的供体硫氢化钠(Na HS),对照组给予相同剂量的生理盐水,术后2、4周分别切除移植静脉,计算机图像分析系统测量和计算内膜、中膜厚度,用免疫组织化学方法检测静脉移植物平滑肌细胞增殖细胞核抗(PCNA)的表达,统计分析所有大鼠内膜厚度和内膜截面积、PCNA指数情况。结果两组静脉移植物在切取时均保持通畅,无血栓和闭塞,其中实验组和对照组静脉移植物的内膜和中膜的厚度在术后增加明显,而实验组内膜、中膜厚度与对照组比较,在术后2、4周增加较轻,差异有统计学意义(P<0.05);两组静脉移植物在切取时PCNA阳性细胞主要分布于静脉移植物中膜近管腔侧与内膜,其中实验组和对照组PCNA指数在术后降低明显,而实验组内膜、中膜厚度与对照组比较,在术后2、4周降低较轻,差异有统计学意义(P<0.05)。结论外源性硫化氢可有效抑制主动脉平滑肌细胞的增殖和促进其凋亡,抑制静脉移植物内膜的增生,有利于预防静脉桥再狭窄的发生,值得临床进一步推广。
Objective To investigate the effect of hydrogen sulfide on intimal hyperplasia in the rat vein grafts.Methods External carotid vein transplantation model of rats was established,24 male rats were randomly divided into the control group and experimental group,each group of 12,the experimental group was given the administration of the hydrogen sulfide donor NariS,control group was given the same dose of normal saline,2 weeks and 4 weeks after excision of transplanted vein respectively,computer image analysis system was used to measure intimal and medial thickness in the endometrium was calculated,mmunohistochemical method was used to detect vein graft smooth muscle cell proliferation nucleus (PCNA) expression,statistical analysis of all rats intimal thickness and intimal sectional area,the PCNA index was given.Results Two groups of VG were kept unobstructed,at the time of taking no thrombosis and occlusion,the ex- perimental group and control group of vein grafts in endometrium and the thickness of the membrane after surgery were significantly increased,while the experimental group in the endometrium,compared with control group,intimal thickness 2 weeks and 4 weeks after surgery was increased lighter,with significant difference (P〈0.05);the two groups of VG in the PCNA positive cells mainly distributed in near the lumen side and inner membranes in VG PCNA index after surgery,in the experimental group and control group were reduced obviously,and compared with control group ,the experimental group in the endometrium,intimal thickness,2 weeks and 4 weeks after surgery was reduced lighter,with significant difference (P〈0.05).Conclusion Exogenous hydrogen sulfide can effectively restrain the aortic smooth muscle cell proliferation and promote its apoptosis,inhibition of vein graft intimal hyperplasia, it is advantageous to the prevention of the occurrence of restenosis bridge,it is worth clinical for further promotion.
