Effect of Blood Glucose Control in Pregnant Women with Gestational Diabetes Mellitus on Cardiopulmonary Function of Newborns

摘要 目的探讨妊娠期糖尿病(gestational diabetes mellitus,GDM)产妇血糖控制情况对新生儿肺功能及心脏功能的影响。方法对2013年1月—2014年12月收治的GDM 109例的临床资料进行回顾性分析,所有产妇入院后均接受血糖干预治疗,根据血糖控制效果分为观察组(产前血糖恢复至正常水平)51例和对照组(产前血糖未恢复至正常水平)58例。测量两组新生儿出生1周后肺功能与心脏收缩、舒张指标。结果观察组新生儿潮气量、潮气量与、潮气呼气峰流速值高于对照组(P<0.05),呼吸频率、吸呼比、功能残气量、气道阻力值低于对照组(P<0.05);观察组新生儿主动脉峰速、肺动脉峰速值低于对照组(P<0.05),左心室射血分数、左心室缩短分数、左心/右心输出量与对照组比较差异无统计学意义(P>0.05);观察组新生儿二尖瓣E/A峰速比与对照组比较差异无统计学意义(P>0.05),而三尖瓣E/A峰速比低于对照组,上腔静脉前负荷指数高于对照组(P<0.05)。结论对GDM产妇进行合理有效的血糖干预,可减轻高血糖对胎儿心肺功能的损害,利于提高新生儿的生存质量。 Objective To investigate effect of blood glucose control in pregnant women with gestational diabetes mellitus ( GDM) on cardiopulmonary function of newborns. Methods Clinical data of 109 GDM pregnant women admit-ted during January 2013 and December 2014 was retrospectively analyzed, and all the patients received intervention ther-apy of blood glucose and were divided into observation group (n=51, prenatal blood glucose return to normal level) and control group (n=58, prenatal blood glucose did not return to normal level) according to control conditions of blood glu-cose. Pulmonary function and cardiac systolic and diastolic indexes were detected 1 week after birth in the two groups. Results Compared with those in control group, in observation group, values of tidal volume (TV), TV per kg body weight and peak expiratory flow rate (PEFR) of tidal air were significantly higher (P〈0. 05);values of respiratory rate, inspiratory/expiratory ratio, functional residual capacity and airway resistance were significantly lower (P〈0. 05); val-ues of aortic peak velocity and pulmonary artery of peak velocity in newborns were lower (P〈0. 05), while there were no significant differences in left ventricular ejection fraction, left ventricular fractional shortening and left/right cardiac out-put (P〉0. 05);there was no significant difference in mitral valve E/A peak velocity ratio of newborns (P〉0. 05), while value of tricuspid valve E/A peak velocity ratio was significantly lower, and value of precava preload index was sig-nificantly higher (P〈0. 05). Conclusion Reasonable and effective intervention of blood glucose in pregnant women with gestational diabetes mellitus can alleviate damage of fetal cardiopulmonary function induced by hyperglycemia, and it is conductive to improving quality of life of newborns.
