摘要
目的研究胰腺癌并糖尿病患者行胰十二指肠切除术后并发症的发生情况,探讨糖代谢对其的影响,为临床治疗提供依据。方法选取2006年1月至2015年5月行胰十二指肠切除术的246例胰腺癌患者进行研究。其中合并Ⅱ型糖尿病患者117例为观察组,无糖尿病患者129例为对照组,分析2组行胰十二指肠切除术后并发症及糖代谢的情况。结果术后空腹血糖及糖化血红蛋白水平得到了明显改善;观察组胰漏发生率较对照组低(P<0.05),差异具有统计学意义。结论胰腺癌并糖尿病患者行胰十二指肠切除术后血糖水平可得到改善,并可降低胰漏的发生率。
Objective To study the occurrence of complications after pancreaticoduodenectomy and the effect of sugar metabolism on complications in pancreatic cancer patients with diabetes mellitus,and to provide a basis for clinical treatment.Methods A total of 246 patients who underwent pancreaticoduodenectomy for pancreatic cancer between January 2006 and May 2015 were selected in this study.These patients were divided into two groups:117 patients with type 2 diabetes(observation group)and 129 patients without diabetes(control group).The complications and glucose metabolism were observed after pancreaticoduodenal resection.Results The levels of fasting blood glucose and glycosylated hemoglobin were significantly improved after operation. Compared with control group,the incidence of pancreatic leakage significantly decreased in observation group(P 〈0.05).Conclusion Pancreaticoduodenectomy can improve the level of blood glucose and reduce the incidence of pancreatic leakage in pancreatic cancer patients with diabetes mellitus.
