关节镜下同种异体韧带与自体肌腱重建前交叉韧带中期疗效比较 被引量:1

Comparison of medium-term efficacy of arthroscopic reconstruction of anterior cruciate ligament by allogeneic ligament and autogenous tendon

摘要 目的对关节镜下行同种异体韧带与自体肌腱进行前交叉韧带(ACL)重建的中远期临床疗效作出评估。方法对86例膝关节ACL病例进行回顾性分析,按手术方法不同分为A组和B组。其中A组应用同种异体韧带,共48例:B组应用自体肌腱重建前交叉韧带.共38例。对比两种方法术后1~5年期间患者膝关节功能评估(Lysholm评分和膝关节疼痛程度)和膝关节客观评定。结果术后2年,两组的膝关节功能评估的Lysholm评分评分结果有明显差异性(P〈0.05)。术后膝关节疼痛程度评价结果显示,术后2年及3年B组患者主观疼痛评分均高于A组(P〈0.05)。术后2年膝关节疼痛客观评定结果显示,屈伸受限及稳定性试验B组优于A组(P〈0.05)。结论自体肌腱重建前交叉韧带中期疗效较同种异体韧带好,根据情况可优先选择。 Objective To evaluate the clinical medium and long-term efficacy of arthroscopic reconstruction of anterior cruciate ligament (ACL) by allogeneic ligament and autogenous tendon. Methods 86 cases of ACL were retrospectively analyzed and divided into group A and B as the differences of the surgeries. Allogeneic ligament was applied in the pa- tients from group A with 48 cases and autogenous tendon was adopted to reconstruct the ACL in the patients from group B with 38 cases. Functional evaluation of knee joint (Lysholm scoring and pain degree of knee joint) and objec- tive evaluation of knee joint of the patients within 1-5 years after the two surgeries was compared. Results Significant differences were presented in the Lysholm scoring of the patients from both groups 2 years after the surgeries (P〈0.05). The objective evaluation of pain of patients from group B were higher than those of group A 2 and 3 years after the surgeries shown by the pain degree evaluation of the knee joint (P〈0.05). The result of flexion and extension restriction and stability test of group B were better than those of group A (P〈0.05). Conclusion The medium-term effect of recon- struction of anterior cruciate ligament by autogenous tendon is better than the allogeneic ligament, which can be chosen according to the specific situation.
