摘要
目的：探讨鼻腔鼻窦解剖变异与慢性鼻窦炎的关系，分析鼻内镜鼻窦手术中鼻腔解剖变异处理措施。方法选取2013年1月到2015年1月于本院就诊的慢性鼻窦炎并伴有鼻腔鼻窦解剖变异患者共86例，给予患者鼻内镜鼻窦手术中鼻腔解剖变异治疗，观察患者术后治疗效果。结果本组痊愈72例，好转9例，无效5例，总有效率为94．2%（81/86）。数据显示，钩突角度随窦内软组织增厚而减小，其中同侧上鼻窦软组织厚度0分组和3分组的钩突角度差异具有显著性（P〈0．05），表明钩突越向内偏曲趋于水平，越容易患上鼻窦炎。其中Ⅰ型总有效率为100．0%（60/60），Ⅱ型为85．0%（17/20），Ⅲ型为66．7%（4/6），Ⅲ型总有效率显著低于Ⅰ型，差异有统计学意义（χ2=7．14，P〈0．01）。结论临床上在给予鼻窦炎患者鼻腔解剖手术治疗时，应该在术前和术中以及术后，观察患者是否存在解剖变异现象，对相关解剖变异患者应该在鼻内镜下及时纠正和处理，能提高患者鼻部通气情况，避免引起慢性鼻窦炎等并发症，提高治疗效果，减少术后复发率。
Objective To investigate the relationship between nasal anatomic variation and chronic sinusitis, analysis the anatomic variations and treatment measures in the endoscopic sinus operation. Methods From January 2013 to January 2015 in our hospital, 86 patients with chronic sinusitis and nasal cavity and paranasal sinuses were given endoscopic sinus operation to treat the anatomic variations of the nasal cavity, and the treatment effect were observed. Results In this group, 72 cases were cured, 9 cases improved, 5 cases ineffective, the total effective rate was 94.2%（81/86）. The effective rate of type1 was 100%（60/60）, type2 was 85%（17/20）, type3 was 66.7%（4/6）. The efficiency of type3 was significantly lower than that of type1, the difference was statistically significant （ P〈0.01）. Conclusion In clinical, when patients with sinusitis treated with operation, anatomical variation of the nasal cavity should be evaluated before operation and intraoperation and postoperation, and should be corrected under nasal endoscope, and which can improve the patients with nasal ventilation, avoid causing chronic sinusitis complications, improve treatment effect, reduce the recurrence rate.
