摘要
延续护理对高血压患者的健康维护具有重要作用。本文从延续护理的形式、内容和效果评价进行综述,旨在为高血压患者延续护理的临床实践提供参考。
Transitional care plays an important role in the health of patients with hypertension.This article reviews the content, form and effect of the continuous care of hypertension, in order to provide references for the further implementation of continuing care for hypertension patients.
