老年腹膜透析患者血清脑钠肽与N-末端脑钠肽前体水平的临床意义分析 被引量:1

摘要 目的探讨老年腹膜透析患者血清脑钠肽(BNP)与N-末端脑钠肽前体(NT-pro BNP)水平的临床意义。方法将简阳市人民医院2009年4月至2012年4月行腹膜透析治疗的60例老年患者纳入观察组,选取同期于本院体检的30例健康老年人纳入对照组。采用酶联免疫吸附测定(ELISA)检测并比较两组研究对象血清BNP及NT-pro BNP水平。所有入组患者均随访至2015年4月。探讨血清BNP及NT-pro BNP水平与患者预后的关系。结果 60例患者平均随访(41.5±7.8)个月,随访期间12例(20%)死亡。观察组患者血清BNP、NT-pro BNP水平均明显高于对照组(P<0.05);与血清BNP、NT-pro BNP低水平患者相比,血清BNP、NT-pro BNP高水平患者的年龄较大,透析时间较长,收缩压高,且高脂血症、心血管疾病发生率及死亡率均较高(P<0.05)。NT-pro BNP水平是老年腹膜透析患者死亡的独立危险因素(HR=2.35,P<0.05)。结论老年腹膜透析患者血清BNP与NTpro BNP水平明显高于正常老年人群,高水平NT-pro BNP是患者死亡的独立危险因素。 Objective To explore the clinical significance of serum brain natriuretic peptide(BNP) and N-terminal brain natriuretic peptide(NT-pro BNP) in elderly peritoneal dialysis patients. Method 60 elderly patients who underwent peritoneal dialysis in Jianyang People's Hospital from April 2009 to April 2012 were selected as observation group and 30 healthy elderly people were selected as control goup. The levels of serum BNP and NT-pro BNP were determined by enzyme-linked immunosorbent assay(ELISA). The relationship of BNP and NT-pro BNP levels with the prognosis of observation group patients were detected. All the patients were followed up to April 2015. Result The average follow-up time of 60 patients was(41.5±7.8) months, 12 cases(20%) died during the follow-up period. The NT-pro BNP and BNP levels in observation group were significantly higher than control group(P〈0.05). Compared to patients with lower BNP and NT-pro BNP levels, the age, dialysis age, systolic blood pressure, hyperlipidemia, incidence of cardiovascular disease and mortality were significantly higer than patients with high BNP and NT-pro BNP levels(P〈0.05). NT-pro BNP level was an independent risk factor for predicting the death of peritoneal dialysis patients(HR = 2.35, P〈0.05). Conclusion The levels of NT-pro BNP and BNP are significantly higher in elderly peritoneal dialysis patients than in healthy people, and high NT-pro BNP level is an independent risk factor for mortality.
