摘要
目的探讨新疆某三级甲等综合性医院包虫病患者住院费用的影响因素,为医疗费用控制提供参考依据。方法选择某医院2010年-2014年之间因包虫病住院的患者为研究对象,收集患者住院费用及相关资料,使用多重线性及通径分析从多因素的角度分析其医疗费用的影响因素。结果经多重线性回归分析,包虫病患者住院费用的影响因素回归方程为:Y=3.272+0.018X_9+0.001X_2+0.077X_4+0.549X_(10)-0.089X_5+0.497X_6+0.003X_(11)(R^2=0.75,F=326.29,P=0.000);对住院费用的通径分析显示,该模型对包虫病住院费用的总解释度为75%;通过对各因素作用系数比较得知,影响包虫病住院费用的因素包括否手术、住院天数、药费比、转科、其他诊断条目、年龄等(P均<0.05)。结论本次研究分析显示手术及住院天数是包虫病患者住院费用的主要影响因素。该研究可为分级阶梯式的包虫病补助政策和该病种DRGs分型提供参考依据,以进一步推动新疆地区的包虫病防治工作。
Objective To study the influencing factors the hospitalization cost of 2064 cases of patients with echinococcosis in a Class A and Tertiary Comprehensive Hospital in Xinjiang, provide reference for the containment of hospitalization cost. Methods To select patients with echinococcosis who discharged between 2010 and 2014 of a hospital, collect related materials of hospitalization cost, analyze the influencing factors of medical expenses with the application of multiple linear and path-coefficient analysis methods as well as from the anglemultifactor analysis.Results By the multiple linear regression analysis,the regression equation on the hospitalization expenses influence factors of echinococcosis patients was Y=3.272＋0.018X_9＋0.001X_2＋0.077X_4＋0.549X_（10）-0.089X_5＋0.497X_6＋ 0.003X_（11）（R2=0.75,F=326.29,P=0.000）.The path analysis of hospitalization cost showed that the total hospitalization expenses for the interpretation of the model of hydatid disease was 75%. By comparing on the various factors effect coefficient, factors that influence the effect of echinococcosis hospitalization costs included surgery,hospitalization, medicines, whether or not the section number, an additional diagnosis, age and so on（P〈0.05）. Conclusions This study analysis showed that whether surgery and hospitalization days were the main factors influencing the cost of echinococcosis patients in the samples of hospital. The research results could provide a reference basis to hierarchical stepwise echinococcosis subsidy policy and the disease of DRGs, further promote the Xinjiang region hydatid disease prevention and control work.
