摘要
目的总结重症脑室出血侧脑室引流术后脑并发症患者的护理经验。方法以2010-01—2015-01我院收治的60例患者为研究对象,所有患者均经头颅CT或MRI检查证实为重症脑室出血,并实施侧脑室引流术。60例患者随机分为观察组与对照组各30例。对照组术后采取常规护理,观察组针对术后脑部并发症,包括术后再出血、脑疝、颅内积气、低颅压、穿刺孔脑脊液漏、颅内感染、中枢性高热等进行综合护理。结果观察组患者术后发生脑疝10例(33.3%),颅内再出血4例(13.3%),颅内感染3例(10.0%),颅内积气5例(16.7%),中枢性高热13例(43.3%),低颅压1例(3.3%),其中观察组患者术后脑疝、颅内再出血、颅内感染、中枢性高热的发生率均显著低于对照组,差异有统计学意义(P<0.05)。结论在对重症脑室出血患者的术后护理中,针对经常出现的脑并发症有的放矢地做好针对性护理,有助于降低术后脑并发症的发生,提高临床疗效。
Objective To ummarize the nursing experience of patients with complications after lateral ventricle drainage of severe ventricular hemorrhage. Methods Sixty patients from Jan. 2010 and Jan. 2010 in our hospital were selected as research objects. All patients were confirmed as severe ventricular hemorrhage through CT or MRI, and received lateral ventricle drain- age. Sixty patients were randomly divided into observation group and control group, with 30 cases in each group. After the operation, the control group received routine nursing, while the observation group received comprehensive nursing targeting complications, including further hemorrhage, cerebral hernia, postoperative intracranial pneumatosis, lower cranial pressure, hole puncture cerebrospinal fluid leak, intracranial infection, and central high fever. Results After operation, there were cere- bral hernia 10 cases （33.3%）, intracranial hemorrhage 4 cases （13.3%）, intracranial infection 3 cases （10.0%）, intracranial pneumatosis 5 cases （16.7%）, neurologic fever 13 cases （43.3%）, and lower cranial pressure 1 case （3.3%） in the observation group. The rates of cerebral hernia, postoperative intracranial hemorrhage, intracranial infection, and neurologic fever of the observation group were significantly lower than those of the control group. The difference was significant （P^0.05）. Conclusion In the postoperative nursing of patients with severe ventricular hemorrhage, targeted nursing to the complications is helpful in reducing the complications and improving clinical effect.
