Galectin-3联合hs-CRP评估慢性心力衰竭患者近期预后的价值被引量：15

摘要目的探讨半乳糖凝集素3（galectin-3）联合超敏C反应蛋白（hs—CRP）评估慢性心力衰竭（CHF）患者近期预后的价值。方法选取CHF住院患者共110例，按美国纽约心脏病协会（NYHA）心功能分级分为Ⅱ～Ⅳ级，检测血清galectin-3、hs—CRP、动态心电图。出院后电话随访90d，记录主要心脏不良事件（MACE）包括因心力衰竭加重再入院或死亡。结果galectin-3、hs—CRP水平均随心功能分级增高而升高（P〈0．05）；不同病因CHF患者间galectin-3、hs—CRP水平比较差异无统计学意义（P〉O．05）；发生MACE组血清galectin-3、hs-CRP水平高于未发生MACE组[（20．0±4．43）ng／mL vs．（17．5±3．47）ng／mL，P〈0．05；（3．38±1．47）mg／L vs．（2．55±1．09）mg／L]；ROC曲线显示galectin-3、hs—CRP预测MACE发生的最佳阈值点分别为22．885ng／mL及4．150mg／L，以阈值点为界，galectin-3增高组恶性心律失常及MACE发生率更高（分别为78．60％725．9．90％及85．71％ vs18．68％，P〈0．05），hs—CRP增高组恶性心律失常及MACE发生率亦更高（分别为70．60％观9．10％及76．47％ vs．18．18％，P〈0．05）。结论galectin-3、hs—CRP与CHF严重程度相关，galectin-3、hs—CRP水平增高的患者近期预后差。
