摘要
目的分析老年股骨颈骨折患者入院时营养状况与手术切口延迟愈合之间的关系,为老年患者临床营养护理提供理论依据。方法根据简易营养评价精法(MNA-SF)评分和糖尿病筛查,将226例股骨颈骨折患者分为营养良好组、营养良好+糖尿病组、营养不良组、营养不良+糖尿病组。研究各组MNA-SF评分与传统营养指标、血清营养学指标的相关性,并分析切口延迟愈合发生率与各指标间的关系。结果术前MNA-SF评分与血清营养学指标所反映的营养评价结果均有良好的相关性。共64例患者发生切口延迟愈合,发生率为28.32%,其中营养良好组切口延迟愈合发生率(3.57%)明显低于营养不良组(33.33%);切口延迟愈合组MNA-SF评分及其评估的营养不良发生率明显低于正常愈合组。结论 MNA-SF营养风险评估是老年人营养状况评价的可靠方法,老年患者营养不良和糖尿病与股骨颈骨折术后切口延迟愈合紧密相关,MNA-SF营养风险评估和血清营养学指标可作为判断患者预后的重要指标。
Objective To analyze the reliability of Mini Nutritional Assessment Short Form( MNA-SF) in evaluating nutritional status,and to observe the effects of nutritional status variation and diabetes on delayed wound healing in the elderly. Methods According to MNA-SF score,226 elderly patients were divided into four groups: wellnourishment group,well-nourishment and diabetes group,malnourishment group,malnourishment and diabetes group. The correlations of total MNA-SF score with conventional nutritional assessing parameters and serological parameters were analyzed. Furthermore,the relationship among delayed wound healing and MNA-SF score was analyzed. Results The preoperative MNA-SF score showed strong correlations with nutritional evaluation results assessed by serological parameters.Sixty-four cases showed delayed healing of postoperative incision. The incidence rate of delayed wound healing in wellnourishment group was 3. 57%(2/56 cases),compared with 8. 33%(3/36 cases) in well-nourishment and diabetes group,33. 33%(27/81 cases) in malnourishment group,60. 38%(32/53 cases) in malnourishment and diabetes group. The MNA-SF score was significantly lower in patients who showed delayed wound healing. Conclusions MNA-SF is a reliable method for evaluating the nutrition status of the elderly patients with femoral neck fractures. The delayed wound healing was strongly associated with nutritional status and diabetes. MNA-SF score can be considered as an important indicator for judging patients' prognosis.
