尿RBP、NAG联合血清CysC检测在2型糖尿病早期肾损伤中的诊断价值 被引量:25

摘要 目的探讨尿视黄醇结合蛋白(RBP)、β-N-乙酰葡萄糖苷酶(NAG)联合血清胱抑素C(CysC)检测在2型糖尿病早期肾损伤中的诊断价值。方法抽取该院T2DM糖尿病肾病患者58例(A组),T2DM单纯糖尿病患者54例(B组),健康体检者52例(C组)。利用免疫比浊法检测3组受试者尿RBP、NAG和血清CysC水平,比较其差异,探讨单项及联合检测诊断糖尿病早期肾损伤的阳性率。结果 A组尿RBP、NAG和血清CysC水平均显著高于B、C组,差异具有统计学意义(P<0.05);B组RBP、NAG和血清CysC水平均高于C组,差异具有统计学意义(P<0.05)。RBP、NAG、CysC单项检测阳性率分别为87.93%、86.21%、84.48%,联合3项指标检测的阳性率为94.83%,高于单项检测的阳性率,差异具有统计学意义(P<0.05)。结论尿RBP、NAG及血清CysC是2型糖尿病早期肾损伤的灵敏指标,且联合3项指标检测的综合敏感性更好,具有较高的临床应用价值。 Objective To explore the diagnostic value of urine retinol binding protein(RBP),beta-N-acetyl amino glycosidase enzymes(NAG)combined with serum cystatin C(CysC)detection in early kidney damage of type 2diabetes mellitus(T2DM).Methods Totally 58 cases of T2 DM complicating in our hospital were selected as the group A,54 cases of simple T2DM(group B)and 52 individuals undergoing the physical examination(group C).The urine RBP,NAG and serum CysC were detected in three groups,their differences were compared.The positive rates for detecting early diabetic renal damage were compared between the single index detection and combined detection.Results The levels of urine RBP,NAG and serum CysC level in the group A were significantly higher than those in the group B and C,the differences were statistically significant(P〈0.05);the levels of urine RBP,NAG and serum CysC level in the group B were significantly higher than those in the group C,the differences were statistically significant(P〈0.05);The positive rates of single detection of RBP,NAG and CysC were 87.93%,86.21%and 84.48%respectively,the positive rate of 3-index combined detection was 94.83%,which was significantly higher than that of single index detection,the difference was statistically significant(P〈0.05).Conclusion Urine RBP,NAG and serum CysC are the sensitive indexes in early diabetic renal damage.The 3-index combined detection has better comprehensive sensitivity and possesses higher clinical application value.
