睡眠呼吸暂停综合征与心律失常的相关性 被引量:4

摘要 目的探讨睡眠呼吸暂停综合征(OSAS)与心律失常间的相关性。方法对48例OSAS合并心律失常病人(OSAS组)和48例单纯鼾症病人(正常组)进行体质量指数(BMI)、呼吸暂停低通气指数(AHI)、血氧饱和度(SaO_2)、心率变异性(HRV)及心率震荡指标进行监测。结果 OSAS组BMI、震荡初始和正常组相比,差异无显著性(P〉0.05);而HRV中的周期标准差平均值、NN50计数占RR周期间数的百分比、震荡斜率以及呼吸暂停低通气指数、SaO_2差异有显著性(t=-32.65~35.69,P〈0.05)。OSAS组病人房性心律失常、室性心律失常、窦性心律失常发生率显著高于正常组(χ~2=17.16~24.08,P〈0.05)。结论 OSAS病人受迷走神经活力降低、交感神经系统活力提高以及自主神经功能失调等因素的影响,导致心律失常的发病率明显提高,临床需引起重视,应及早预防并治疗。 Objective To Investigate the correlation between sleep apnea syndrome(SAS)and arrhythmia. Methods Forty-eight patients with SAS complicating arrhythmia(SAS group)and 48 patients with simple snoring(control group)were enrolled in this study,and the following parameters of patients in both groups were detected:body mass index(BMI),apnea-hypopnea index(AHI),blood oxygen saturation(SaO_2),heart rate variability,and heart rate turbulence. Results The difference between SAS and the control group was not significant in BMI and turbulence onset(P〉0.05),but the differences were significant in terms of standard diviation of NN intervals(SDNN),percentage of total number of sinus beat number of adjacent NN difference〉50ms(PNN50),turbulance slope(TS),AHI,and SaO_2(t=-32.65-35.69,P〈0.05).Incidences of atrial arrhythmia,ventricular arrhythmias and sinus arrhythmia in SAS group were significantly higher than that in the control group(χ~2=17.16-24.08,P〈0.05). Conclusion Because of low activity of nervi vagus,high activity of autonomic nerves and autonomic nervous dysfunction,the incidence of arrhythmia in patients with sleep apnea syndrome increases,an attention to this condition needs to be paid for an early prevention and treatment.
