早期气管切开对颅脑外伤合并出血患者脑氧代谢及神经功能的影响研究 被引量:9

Study on the Influence of Early Tracheotomy for the Cerebral Oxygen Metabolism and Nerve Function in Patients with Craniocerebral Trauma Combined with Hemorrhage

摘要 目的:观察早期气管切开对颅脑外伤合并出血患者脑氧代谢及神经功能的影响程度。方法:选取2014年2月至2015年7月于本院进行治疗的62例颅脑外伤合并出血患者为研究对象,将其随机分为对照组31例和观察组31例,对照组进行常规颅脑外伤合并出血治疗,观察组则在此基础上加用早期气管切开治疗,然后将两组患者治疗前及治疗后不同时间的脑氧代谢指标及血清神经功能、氨基酸类神经递质指标进行比较。结果:两组患者治疗前的脑氧代谢指标及血清神经功能、氨基酸类神经递质指标均无显著性差异,P均>0.05,而治疗后不同时间观察组的检测结果均好于对照组,P均<0.05,均有显著性差异。结论:早期气管切开对颅脑外伤合并出血患者脑氧代谢及神经功能的影响更为积极,对于患者的综合状态改善具有积极的临床价值。 Objective: To observe the influence degree of early tracheotomy for the cerebral oxygen metabolism and nerve function in patients with craniocerebral trauma combined with hemorrhage. Method: 62 cases with craniocerebral trauma combined with hemorrhage from February 2014 to July 2015 in our hospital were selected as the objects in this research. All patients were randomly divided into two groups: control group and observation group,with 31 cases in each group. The control group received conventional treatment for craniocerebral trauma combined with hemorrhage,while early tracheotomy was used in the observation group on that basis. Then,the indexes of cerebral oxygen metabolism,serum nerve function and amino acid neurotransmitters at different times were compared between two groups before and after treatment. Result:Before treatment,there was no significant differences in the cerebral oxygen metabolism index,serum nerve function and amino acid neurotransmitters index between the two groups( P〈0.05). However,the detection results of observation group at different times were much better than those of control group after treatment( P〈0.05). Conclusion: The application of early tracheotomy exerted more positive influence on the cerebral oxygen metabolism and nerve function in patients with craniocerebral trauma combined with hemorrhage. And it has good clinical value for the improvement of comprehensive states of patients.
