摘要
目的总结颅咽管瘤术后防治水钠代谢紊乱的护理措施。方法回顾性分析2010-2013年北京天坛医院神经外科138例颅咽管瘤术后出现水钠代谢紊乱不同症状患者的临床资料。结果出现水钠代谢紊乱的138例患者中,119例行肿瘤全切术,19例行肿瘤次全切除术;尿崩症101例,高钠血症35例,低钠血症52例,交替性血钠异常27例。通过严密观察生命体征,准确记录出入量,及时发现病情变化、明确原因,及早纠正各种血钠紊乱等,138例患者中临床治愈135例,好转3例。结论术后严密监测出入水量,血电解质变化,观察患者意识和症状的改变,出现症状时准确辨别并及时采取措施可有效防治水钠代谢紊乱。
Objective To summarize the early management of water and sodium disturbance after craniopharyngioma surgery. Methods One hundred and thirty-eight patients who had a surgery for craniopharyngioma resection were analyzed retrospectively for postoperative disturbance of water and sodium. Results Among these 138 cases, 119 were total resected, 19 were subtotal resected. One hundred and one cases had diabetes insipidus, 35 had hypernatremia, 52 had hyponatremia and alternated with hyper or hypo-natriemia. Through observing the vital signs intensively, recording the intake and output volume accurately, discovering the condition changes in time and clearing the reason, and correcting all kinds of serum sodium disorders early,135 cases recovered clinically and 3 cases improved. Conclusion Postoperative intensive monitoring of water intake and discharges, fluctuation of electrolytes, and observing patients＇ consciousness level and complaints, correctly distinguishing the symptoms and taking proper measures can prevent and treat the disturbance of water and sodium abnormalities effectively.
