摘要
目的探讨采用高位结扎剥脱大隐静脉主干联合泡沫剂硬化属支治疗下肢静脉曲张的疗效与经济性分析。方法2012年3月至2014年3月在潍坊市人民医院血管外科住院的下肢静脉曲张患者121例,按随机数字表法分为两组,对照组采用常规高位结扎剥脱大隐静脉主干辅助以属支点式剥脱的术式进行治疗,干预组在此基础上将属支点式剥脱改为属支超声引导下泡沫剂注射硬化方式进行治疗。评价两组患者的手术情况、术后疼痛评分、术后恢复等数据,记录患者住院天数,统计手术及术后1年因治疗下肢静脉曲张产生的费用。结果干预组患者的平均手术时间、术中出血量、术后疼痛评分、住院时间、术后恢复情况均明显好于对照组(P<0.05),第一次住院平均费用、总费用低于对照组(P<0.05),两组并发症差异无统计学意义(P>0.05)。结论采用高位结扎剥脱大隐静脉主干联合泡沫剂硬化属支治疗下肢静脉曲张的疗效好于传统术式,费用也低于传统术式,具有较好的临床推广价值。
Objective To investigate the efficacy of high ligation and stripping off trunk of great saphenous vein and foam sclerotherapy in the treatment of varicose veins of lower limbs and perform the economic analysis.Methods One hundred and twentyone patients with varicose veins of lower limbs from March 2012 to March 2014 treated at our department in Weifang People＇s Hospital were randomly divided into control or intervention group.The control group was treated with conventional high ligation and stripping of great saphenous vein trunk that was an operation of fulcrum type stripping.In the intervention group,the operation of fulcrum type stripping was changed to ultrasound guided foam sclerotherapy.The operation conditions,postoperative pain score and postoperative recovery data were compared between the two groups.The days of hospitalization were recorded.The cost for treatment including operation and one year after operation were compared between the two groups.Results The average operative time,intraoperative bleeding volume,postoperative pain score,time of hospitalization and postoperative recovery in the intervention group were all better than that of the control group（ P〈 0. 05）.The average cost of intervention group was lower than that of the control group（ P 0. 05）.Conclusion The effect of high ligation and stripping of great saphenous vein and foam hardening for the treatment of varicose veins of lower limbs is better than the traditional operation,and the resulting costs is also lower than the traditional operation. Therefore,it has a good clinical application value.
