摘要
目的探讨后腹腔肾脏切除术治疗肾脏肿瘤的临床效果。方法 46例肾脏肿瘤患者分为观察组21例,采用后腹腔肾脏切除术治疗;对照组25例,采用开放肾切除术。分别观察2组患者手术时间、术中出血量、术后胃肠道功能恢复时间、下床时间、引流管留置时间、住院时间、使用镇痛药物病例数及并发症发生情况。同时于术前24 h和术后第1,3,5 d采静脉血进行白细胞(WBC)计数及血清C-反应蛋白(CRP)测定。2组患者均于术后第1个月进行随访。结果观察组手术时间、术中出血量、术后胃肠道功能恢复时间、下床时间、引流管留置时间及住院时间分别与对照组比较,差异均有统计学意义(P<0.01)。观察组使用镇痛药物病例数少于对照组。观察组术后第1,3,5天WBC及CRP含量分别与对照组比较,差异有统计学意义(P<0.05)。结论后腹腔肾脏切除术治疗肾脏肿瘤是一种创伤小、恢复快、安全可靠、疗效确切的手术方法。
Objective To explore the clinical effect of retroperitoneal nephrectomy treating renal tumors. Methods46 patients with renal tumors are divided into two groups according to operation ways,namely,21 patients in observation group,treated by retroperitoneal nephrectomy,and 25 patients in control group,treated by open nephrectomy.Respectively observe the two groups＇ operation time,intraoperative blood loss,postoperative gastrointestinal function recovery time,postoperative out-of-bed time,postoperative drainage tube indwelling time,postoperative hospital stay time,patient number of using analgesic drug and complications. At the same time,collect venous blood in preoperative 24 h and postoperative 1,3,5d,to count white blood cell and determine serum C-reactive protein. The two groups＇ patients are followed up in postoperative 1 month.Results Respectively compare the operation time,intraoperative blood loss,postoperative gastrointestinal function recovery time,postoperative out-of-bed time, postoperative drainage tube indwelling time and postoperative hospital stay time of observation group with what of control group,the difference has statistical significance（P〈0.01）. Compare the patient number of using analgesic drug of observation group with what of control group. Respectively compare postoperative WBC of 1,3,5d and CRP content with what of control group,the difference has statistical significance（P〈0.05 or P〈0.01）. Conclusion For the treatment of renal tumors,retroperitoneal nephrectomy is the operation method with small trauma,quick recovery,safe,reliable and with exact effect.
