摘要
目的:观察针灸联合康复训练治疗脑梗死后偏瘫患者日常生活能力及肢体运动功能的影响。方法:将116例脑梗死后偏瘫患者随机分为对照组和治疗组患者各58例。对照组患者在西医常规治疗的基础上给予康复训练,治疗组患者则联合针灸治疗,两组均以4周为1个疗程。用Barthel指数及Fugl-Meyer评分分别对治疗前后患者的日常生活能力及肢体运动功能进行评定;用神经功能缺损程度评分量表(NIHSS)对两组患者治疗前后的神经功能缺损程度进行评分;观察治疗后患者肌力的变化情况,比较两组治疗的有效率。结果:两组患者经治疗后日常生活能力及肢体运动功能较治疗前有所提高,Barthel指数及Fugl-Meyer评分有所上升,治疗组患者的改善情况较对照组更明显,差异有统计学意义(P<0.05);患者经治疗后神经功能有所提高,神经功能缺损程度评分NIHSS较治疗前下降明显,但以治疗组改善更为突出,差异有统计学意义(P<0.05);患者经治疗后肌力提高明显,以治疗组的改善更突出,差异有统计学意义(P<0.05);治疗组的有效率为91.3%,明显高于对照组治疗的有效率72.4%,差异有统计学意义(P<0.05)。结论:针灸联合康复训练治疗脑梗死后偏瘫患者疗效显著,能有效提高患者的日常生活能力及肢体运动功能,改善患者神经功能,凸显出中西医结合外治法论治脑梗死后遗症的优势与特色,值得临床深入研究运用。
Objective： To research the effect of applying acupuncture combined with rehabilitation training in the treatment of hemiplegia after stroke on daily life ability and limbs movement functions. Methods： 116 patients with hemiplegia after stroke were randomly divided into a controlled group of 58 cases and a treatment group of 58 cases from March 2012 to October 2014. Patients in the controlled group were treated with rehabilitation training on the basis of conventional western medicine treatment; while patients in the treatment group were treated with acupuncture,4weeks for treatment. We evaluated the daily life ability and limbs movement functions with Barthel Index and Fugl- Meyer Score,and appraised the degree of nervous functional defects with NIHSS. Finally,we observed the change of patients＇ muscle strength and calculated the effective rate of the two groups. Results： After treatment,the daily life ability and limbs movement functions of the patients in two groups were improved clearly compared with before treatment,the Barthel Index and Fugl- Meyer Score went up,the degree of change in the treatment group was more pronounced,the difference was statistically significant（ P〈0. 05）; the nerve function of the patients in two groups were improved clearly compared with before treatment,while the NIHSS score went down significantly,the degree of change in the treatment group was more pronounced,the difference was significant（ P〈0. 05）; the patients＇ muscle strength in two groups were improved clearly,the degree of change in the treatment group was more pronounced,the difference was significant（ P〈0. 05）;the efficiency for patients in the treatment group was 91. 3%,which was higher than that of the controlled group 72. 4%,the difference is significant（ P〈0. 05）. Conclusion： Treating patients with hemiplegia after stroke with acupuncture combined rehabilitation training has good effects. The therapy can improve the patients＇ daily life ability,limbs movement functions and nerve function,which shows the advantages and characteristics of treating cerebral infarction sequel with traditional Chinese and western medicine,is worthy of further research and development.
