摘要
目的:研究近视患者近视发病时间(T)对黄斑区脉络膜厚度(CT)的影响。方法:采用前瞻性队列研究方法。纳入2014-03/2015-04间在我院屈光手术中心行手术前检查的122例244眼近视患者。其中,男67例,女55例。年龄30~41(平均35.1±4.6)岁。根据发病时间分成3组:A组(T≤5a)、B组(5a0.05)、年龄(F=2.13,P〉0.05)、最佳矫正视力(F=1.41,P〉0.05)、角膜曲率(F=1.65,P〉0.05)、眼轴长度(F=1.89,P〉0.05)差异均无统计学意义。受检查者黄斑区脉络膜使用频域光相干断层扫描(SD-OCT)的深度增强成像(EDI)技术进行扫描。测量中心凹下CT(SFCT)以及距中心凹1、3mm处上方(S)、下方(I)、颞侧(T)、鼻侧(N)4个方位的CT。备方位点分别标记为S_(1mm)、I_(1mm)、T_(1mm)、N_(1mm)、S_(3mm)、I_(3mm)、T_(3mm)、N_(3mm)。分析三组间脉络膜相同位点CT值的差异。结果:A、B、C组的平均SFCT值分别为238.32±57.95、230.58±67.21、221.63±62.37μm。A、B组间及B、C组间除N_(3mm)外(t_(A-B)=4.34、t_(B-C)=7.61,P〈0.05),其余各位点的CT值差异均无统计学意义(P〉0.05);A、C组间除T_(3mm)外(t=0.76,P〉0.05),其余各位点的CT值差异均有统计学意义(P〈0.05);三组间两两比较,N_(3mm)的CT值差异均有统计学意义(t_(A-B)=4.31、t_(B-C)=7.59、t_(A-C)=12.18,P〈0.05)。结论:随着近视发病时间的延长,近视患者的脉络膜厚度变薄鼻侧尤为明显。
AIM:To investigate the effect of onset time(T) for macula choroidal thickness(CT) in myopia patient.·METHODS:A prospective cohort study was designed.One hundred and twenty-two myopia patients(244 eyes;67 male and 55 female;aged 30~41 years,mean 35.1±4.6years old) who received preoperative examinations from March 2014 to April 2015 were recruited in this study.The patients were divided into three groups according to onset time(T):group A(T ≤ 5a),group B(5aT≤10a) and group C(10a T ≤15a).There were no significant differences on equivalent power(F=1.56,P0.05),age(F=2.13,P0.05),best corrected visual acuity(BCVA,F=1.41,P0.05),corneal curvature(F= 1.65,P0.05) and axial length(F= 1.89,P0.05) among the three groups.The choroid in macular region was measured by enhanced depth imaging(EDI) using spectral-domain optical coherence tomography(SD-OCT).This study recorded the CT at subfoveal(SFCT),1mm at temporal(T_(1mm)),nasal(N_(1mm)),superior(S_(1mm)) and inferior(l_(1mm)) to the fovea and 3mm temporal(T_(3mm)),nasal(N_(3mm)),superior(S_(3mm)) and inferior(l_(3mm)) to the fovea,respectively.The differences of CT at the same position among the three groups were analyzed.·RESULTS:The mean SFCT for group A,B,C were 238.32±57.95μm,230.58±67.21μm,221.63±62.37μm respectively in this study.The CT was found no significant difference in different locations except N_(3mm)(t_(A-B) = 4.34,P0.05)between group A and B.The CT was found no significant difference in different locations except N_(3mm)(t_(B-C) = 7.61,P0.05) between group B and C.The CT was found significant difference in different locations,except T_(3mm)(t=0.76,P0.05) between group A and C.Significant difference was found at N_(3mm)(t_(A-B)=4.31,t_(B-C) = 7.59,t_(A-C) =12.18;P0.05) among the three groups.·CONCLUSION:The choroidal thickness decreases as the myopia onset time is earlier,especially at nasal.
