摘要
目的探讨腹腔镜下经膀胱入路膀胱阴道瘘修补术的手术方法、安全性及疗效。方法回顾性分析我院2011年7月-2015年7月单一术者所行的腹腔镜下经膀胱入路膀胱阴道瘘修补术21例的资料,分析手术时间、术中出血量、手术成功率、术后住院天数、并发症发生率等指标。结果 21例手术均获成功,无中转开放手术。手术时间为115-180 min,平均（125±25.0）min;术中出血量为60-280 ml,平均（130±65.0）ml;术后平均住院日为5-9 d,平均（6.5±2.0）d;术后留置尿管为14-21 d,平均（18±4.0）d。术中、术后均无输血,无漏尿、输尿管及肠管损伤等并发症。随访3-45个月,平均（25±20）个月,18例治愈,2例好转,1例失败。结论腹腔镜下经膀胱入路膀胱阴道瘘修补术安全、有效,创伤小,恢复快,效果较肯定。
Objective To explore the curative effect and safety of bladder approach vesicovaginal fistula neoplasty under laparoscope.Methods Selected 21 cases with VVF undergoing transvesical laparoscopic repair from July 2011 to July 2015 by one surgeon were reviewed retrospectively,analyzed the data on the operation time,the blood loss during operation,the operation successful rate,the mean hospitalization time after operation,the complications.Results 21 patients were successfully operated,no cases were transferred to open surgery.The operation time was 115 to 180 minutes,with an average of（125±25.0） minutes.The blood loss during operation was 60 to 280 ml,with an average of（130±65.0） ml.The hospitalization time after operation was 5 to 9 days,with an average of（6.5±2.0） days.The catheter indwelling time was 14 to 21 days,with an average of（18±4.0） days.No blood transfusion occurred,no major intraoperative and postoperative complications such as urine leakage,ureteral damage and intestinal injury occurred.With range 3 to 45 months,mean follow-up of（25±20）months,18 cases cured,2cases improved and 1casefailed.Conclusion Intravesical laparoscopic repair of VVF with the great omentum shift and conventional layered suture is a safe and efficient treatment.It has the characteristics of less damage,shorter recovery time and positive effect.
