动脉晚期图像在多层螺旋CT泌尿系成像检查诊断中的应用价值

摘要 目的:探讨动脉晚期图像在多层螺旋CT泌尿系成像(MSCTU)检查诊断中的应用价值。方法:搜集近2年间临床资料完整的60例泌尿系疾病患者,均行MSCTU检查,按扫描方法的不同将其分为A组与B组,每组30例。A组采用平扫、皮质期(20~30 s)、髓质期(55~60 s)、排泄期(5~15 min)四期扫描;B组采用平扫、动脉晚期(35~40 s)、排泄期(5~15 min)三期扫描;并应用多种后处理技术观察皮质期、髓质期(A组)与动脉晚期(B组)图像对病变的显示情况和肾血管的评价情况。结果:皮质期、髓质期图像(A组)与动脉晚期图像(B组)均能清晰显示泌尿系病变,检出率无明显差异。肾血管对比剂浓度:A组肾动脉(284±52)HU,肾静脉(123±26)HU;B组肾动脉(252±35)HU,肾静脉(148±27)HU;两组肾动、静脉CT值无差异。结论:MSCTU动脉晚期图像能清晰显示泌尿系病变,同时评价肾动脉及肾静脉,能替代常规皮质期和髓质期图像,并减少辐射剂量和缩短检查时间。 Objective: To investigate the application value of the late arterial phase images in the multi-slice CT urography(MSCTU). Methods: Sixty patients of complete clinical data with diseases of the urography were reviewed retrospectively, they were divided into two groups(group A and B) according to two different scanning methods, each group included 30 cases. Group A used four phases of plain scan, cortical phase(20-30s), medulla phase(55-60s), excretory phase(5-15min); Group B used three phases of plain scan, late arterial phase(35-40s), excretory phase(5-15min). The urography disease and renal vessels could be shown and evaluated through images of cortical phase, medulla phase(A) and the late arterial phase(B) with variety of post-processing techniques. Results: The cortical phase, medulla phase images(A) and the late arterial phase images(B) showed urography disease clearly, the detection rate had no difference; The renal intravascular contrast agent concentration: Group A: renal artery 284±52HU, renal vein 123±26HU; Group B: renal artery 252±35HU, renal vein 148±27HU, No statistically significant differences was found in renal artery and vein. Conclusion: The late arterial phase images showed the lesions of the urinary tract clearly, while observing the renal artery and vein, It can substitute for cortical phase,medulla phase images and reduce radiation dose and scan time.
