摘要
目的 探讨基层医院急性胆源性胰腺炎（AGP）的早期诊断及急诊手术时机.方法 2005年3月至2010年12月收治122例AGP患者,其中对行手术或内镜治疗的AGP33例患者进行回顾性分析,按入院后接受手术或内镜治疗的时机,分为入院后24h内即治疗为早期干预组20例,其它为延迟干预组13例.结果 早期干预组20例全部治愈,切口感染4例,其中合并肺部感染3例;延迟手术组13例中,死亡1例,切口感染及肺部感染6例.结论 在基层医院AGP早期行手术或内镜治疗有助于减少并发症发生率和改善预后.
Objective To explore the early diagnosis and the timing of early surgery for acute biliary pancreatitis in grassroots hospital. Methods 122 patients with acute gallstone pancreatitis for the time period for March 2005 to December 2010 were studied. Within the 122 cases, 33 patients underwent either surgery or endoscopic treatment. Based on the timing of treatment employed, the patients were divided into two groups： early intervention group （ received the treatment within 24 hours of admission, n=20 ） and the control group （ n= 13 ） . Results In the early intervention group, all the 20 patients were cured and discharged from hospital. Among the 20 case, there were 4 cases of incision infection, among which 3 cases complicating pulmonary infection. In the control group, there were one death and 6 cases of incision infection complicating pulmonary infection. Conclusions In grassroots hospital, early surgery or endoscopic treatment for acute biliary panereatitis is beneficial to reducing complication rates and improving patient＇ s outcome.
