摘要
目的从疾病流行病学负担和经济负担两个方面出发,探讨安徽省宣城市精神分裂症疾病负担情况。方法收集2010—2012年于宣城市人民医院、宣城市中心医院和宣城市第四人民医院(宣城市精神病医院)住院、符合纳入与排除标准的全部精神分裂症患者316例为研究对象。计算并比较其疾病流行病学负担指标〔伤残调整寿命年(DALYs)、因早死所致的寿命损失年(YLLs)、疾病所致伤残引起的健康寿命损失年(YLDs)〕和经济负担指标(直接经济负担、间接经济负担)。结果 2010—2012年男、女性精神分裂症患者YLDs均大于YLLs;男性精神分裂症患者YLDs、YLLs均大于女性。2011、2012年次均直接经济负担高于2010年(q=2.77,P<0.05;q=3.11,P<0.05);2012年次均直接经济负担高于2011年(q=2.77,P<0.05)。2010—2012年男性精神分裂症患者次均直接经济负担高于女性(P<0.05)。2010—2012年精神分裂症患者间接经济负担年增长速度为24.21%。2010—2012年男性精神分裂症患者间接经济负担高于女性。结论宣城市2010—2012年精神分裂症疾病负担巨大,且疾病流行病学负担和经济负担均呈逐年上升趋势,提示国家、社会及每个公民均应给予关注和支持,多部门应采取相应措施,遏制宣城市精神分裂症疾病负担进一步加重。
Objective To investigate the disease burden of schizophrenia in Xuancheng of Anhui Province from the aspects of epidemiological burden and economic burden. Methods 316 schizophrenia patients who accorded with inclusion and exclusion criteria and were admitted into Xuancheng People＇ s Hospital,Xuancheng Central Hospital and the Fourth Hospital of Xuancheng（ Xuancheng Psychiatric Hospital） from 2010 to 2012 were enrolled in the study. The indexes of epidemiological burden（ DALYs,YLLs and YLDs） and the indexes of economic burden（ direct economic burden and indirect economic burden） were calculated and compared. Results From 2010 to 2012,the YLDs of male and female schizophrenia patients were higher than YLLs; male schizophrenia patients had higher YLDs and YLLs than female schizophrenia patients. The average direct economic burden in 2011 and 2012 was higher than that in 2010（ q = 2. 77,P〈0. 05; q = 3. 11,P〈0. 05）; the average direct economic burden in 2012 was higher than that in 2011（ q = 2. 77,P〈0. 05）. From 2010 to 2012,the average direct economic burden of male schizophrenia patients was higher than that of male schizophrenia patients（ P〈0. 05）. From 2010 to2012,the annual increase rate of the indirect economic burden of male schizophrenia patients was 24. 21%. From 2010 to 2012,the indirect economic burden of male schizophrenia patients was higher than female schizophrenia patients. Conclusion From2010 to 2012,there was enormous disease burden of schizophrenia in Xuancheng,and the epidemiological burden and economic burden were increasing year by year. We suggest that every citizen should give attention and support in this aspect,and relevant sectors should take corresponding measures,in order to stop the disease burden of schizophrenia from aggravating.
