儿童腺病毒肺炎患者sPLA2、TNF-α、CCSP的变化及临床意义 被引量:4

摘要 目的探讨儿童腺病毒肺炎患者分泌型磷脂酶A2(s PLA2)、肿瘤坏死因子α(TNF-α)、克拉拉细胞分泌蛋白(CCSP)的变化及临床意义。方法根据住院患儿腺病毒肺炎的严重程度将研究对象分为两组,一组为40例6个月至2岁腺病毒-普通肺炎组;一组为45例年龄6个月至2岁腺病毒-重症肺炎组;此外设立40例在儿童保健门诊行健康体检的儿童作为健康对照组。采用ELISA方法分别检测腺病毒-普通肺炎组、腺病毒-重症肺炎组和健康对照组中患儿的血清s PLA2、TNF-α、CCSP水平,并进行比较。结果 1腺病毒-普通肺炎组s PLA2水平为137.25(135.00~147.80)IU/m L,腺病毒-重症肺炎组为138.00(135.00~141.00)IU/m L,分别与正常对照组97.30(96.20~98.30)IU/m L比较有显著差异(P〈0.05);TNF-α、CCSP水平也有显著差异(P〈0.05)。2腺病毒-普通肺炎组与腺病毒-重症肺炎组比较,s PLA2、CCSP水平无明显差异(P〉0.05),而TNF-α有显著差异(P〈0.05)。结论 s PLA2水平在儿童腺病毒肺炎的发病过程中起着重要作用,TNF-α、CCSP水平与s PLA2的表达密切相关。 [ Objective ] To verify if sPLA2, TNF- α ,CCSP is different between pediatric acute severe pneumonia and pediatric pneumonia triggered by respiratory adenoviral virus infection and to clarify how the enzyme may influence the disease severity. [ Methods ] All infants range from 6 months to 2 years old hospitalized for adenoviral pneumonia were enrolled into two groups: one group included 45 infants who developed into acute adenoviral virus severe pneumonia; another group inclued 40 infants with acute adenoviral ordinary pneumonia. We also enrolled 40 health infants as control group. ELISA was used to test sPLA2, TNF- α, CCSP in these samples. [ Results ] 1.The sPLA2 level was 137.25(135.00-147.80) IU/mL in adenoviral ordinary pneumonia group; 138.00(135.00-141.00 )IU/mL in adenoviral severe pneumonia group and 97.30(96.20-98.30) IU/mL in health control group. There was significant differences among these three groups(P〈0.05). There are also significant differences in TNF-α, CCSP level. 2. sPLA2, CCSP levels in adenoviral ordinary pneumonia group and adenoviral severe pneumonia group were not significantly different (P〉0.05), but TNF- α were significantly different (P〈0.05). [ Conclusion ] sPLA2 plays an important role in the pathogenesis of children adenovirus pneumonia, and closely related to the expression of TNF- α, CCSP levels.
