体外冲击波碎石术前是否留置双J管的临床研究 被引量:2

A Randomized and Controlled Study on the Effect of Placement of Double-J Stents before ESWL for Reanl Calculi

摘要 目的探讨体外冲击碎石术(ESWL)治疗肾结石前留置双J管的临床适应证及其必要性。方法按病例纳入标准将160例肾结石患者随机分成两组,行体外冲击波碎石术,A组(80例)治疗前留置双J管,B组(80例)治疗前未留置双J管,术后随访1~3月,比较两组之间的结石清除率及其并发症。结果 A组中9人、B组中6人有〉5mm的结石碎片残留,需二期碎石治疗;3月后结石清除率分别为A组88%,B组91%。术后1~2天两组间肋腹部疼痛,恶心呕吐,短暂性血尿、镇痛剂的使用例数没有显著性差异,随访3月均未出现输尿管狭窄并发症。结论对于直径在1~2cm内的肾结石,ESWL治疗前留置双J管并不利于结石的排出,因此不需要常规术前留置双J管。 Objective To assess the necessity for routine ureteral stenting before extracorporeal shock Wave lithotripsy ( ESWL) for renal calculi .Methods A total of 160 patients with renal calculi amenable to ES-WL were randomized divided into group A ( stented group , n =80 ) or group B ( unstented group , n =80 ) . The stone clearance rate and its complications were compared between the two groups .Results 9 patients in group A,6 patients in group B had 〉5 mm residual fragments..In these patients second ESWL session was necessary.Three months stone-free rate was 88%in group A and 91%in group B, respectively.There was no statistical difference in flank or abdominal pain , nausea, vomiting, transient hematuria, temperature or use of analgesics on the first and second day after ESWL in the stented or unstented group .During 3 months after ES-WL, there was no anatomical ureteral narrowing happened in both groups .Conclusion The use of double-J stents prior to ESWL treatment is not necessity for renal calculi .
