摘要
目的分析门诊头晕患者方药使用的证治规律及不同疾病间的应用差异。方法回顾性分析我院脑病科、耳鼻喉科2010年1月至2014年10月11 868例门诊头晕患者的病历资料,总结不同疾病间的常用方药使用情况。结果 11 868例头晕患者中使用泽泻汤(16.6%)、半夏白术天麻汤(15.9%)较为多见;而温胆汤(1.1%)、黄连阿胶鸡子黄汤(1.1%)较少见。11 868例头晕患者42.2%使用白术且用量5~50 g不等,其中60.3%使用剂量为10 g;35.5%使用泽泻且4~40 g不等,其中61.0%使用剂量为10 g。涉及诊断24项,较为多见的是脑血管病(治疗多用泽泻汤、半夏白术天麻汤)、高血压(治疗多用泽泻汤、天麻钩藤饮)、颈椎病(治疗多用泽泻汤、半夏白术天麻汤)。结论泽泻汤和半夏白术天麻汤在头晕治疗中应用较多,痰饮、痰浊交互致眩较多见。泽泻和白术是治疗头晕常用药物,但临床应用剂量差异较大。
Objective To analyze the rule of prescriptions for dizziness so as to provide guidance for the treatment of dizziness in clinic. Methods 11 868 consecutive outpatients suffering dizziness from January 2010 to October 2010 in Dongzhimen Hospital were involved in this study,the prescriptions used in different diseases were concluded from encephalopathy department and ear-nose-throat( ENT) department. Results In 11 868 outpatients with dizziness,there were 16. 6% patients who were given Zexie decoction and 15. 9% patients given Banxia Baizhu Tianma decoction,and the two prescriptions were used more than others. On the other hand,Wendan decoction( 1. 1%) and Huanglian Ejiao Jizihuang decoction( 1. 1%) were used less. Dose of Baizhu used was 5 ~ 50 g in 42. 2% dizziness patients,including 10 g( 60. 3%); 4 ~ 40 g( 35. 5%). The diagnosis were 24 items,and the common diagnosis were cerebrovascular disease( Zexie decoction and Banxia Baizhu Tianma decoction were used more than others),hypertention( Zexie decoction and Tianma Gouteng yin were used more),cervical spondylosis( Zexie decoction and Banxia Baizhu Tianma decoction were used more than others). Conclusions Zexie decoction and Banxia Baizhu Tianma decoction were more often used in clinic which indicates interaction of phlegm and retained fluid,as well as turbid phlegm are the main cause of dizziness. Zexie and Baizhu are two common herbs which are used in the treatment of dizziness,and different dozes are shown in the study.
