摘要
目的探讨实时荧光定量PCR(RTFQ-PCR)在儿童肺炎支原体(MP)感染中的诊断价值。方法选取106例肺炎患儿,采集静脉血,以间接荧光免疫法(IFA)检测MP-Ig M表达;采集咽拭子标本,以RTFQ-PCR检测MP DNA拷贝数。以血清学检测结果为诊断标准,观察RTFQ-PCR诊断MP的灵敏度、特异度、阳性预测值和阴性预测值,及其在检测不同年龄段患儿MP感染中的准确性;比较MP-Ig M阳性和阴性患儿的MP-DNA拷贝数差异。结果 1 RTFQ-PCR诊断灵敏度、特异度、阳性预测值和阴性预测值分别为90.48%、93.02%、95.00%、89.96%。绘制受试者工作曲线(ROC曲线),曲线下面积为0.740;2 RTFQ-PCR诊断6岁以上患儿MP感染的灵敏度和准确率高于1岁以下患儿(P<0.01);3 MP-Ig M阳性组DNA拷贝数对数显著高于阴性组拷贝数对数(P<0.01)。结论 RTFQ-PCR诊断儿童MP感染具有较好的准确性,其荧光拷贝数越大越支持MP感染的诊断,对发病早期及年龄较大的患儿可能具有更高的诊断价值。
Objective To explore the diagnostic value of real-time quantitative PCR (RTFQ -PCR) in children with mycoplasma pneumoniae (MP) infection. Methods Totally 106 children with pneumonia were chcosed and venous blood was collected. Indirect fluorescence immunoassay (IFA) were applied to detected MP-IgM expression. RTFQ-PCR was used to determine the MP DNA copies collected from throat swab specimens. Diagnostic sensitivity, specificity, positive pre- dictive value and negative predictive value of RTFQ-PCR as well as detection accuracy among differ- ent ages were observed according to diagnostic criteria serological test. Meanwhile the differences of MP-DNA copies between MP-IgM positive and negative children was compared. Results (1) Diag- nostic sensitivity, specificity, positive predictive value and negative predictive value of RTFQ-PCR were 90.48 %, 93.02 %, 95.00 %, 89.96 % respectively. Receiver operating characteristic curve was analysed and the area under the curve was 0. 740. (2) Sensitivity and accuracy of RTFQ-PCR in MP children over 6 years old were higher than in children under 12 months (P〈0.01). (3) Loga- rithm of DNA copies in MP-IgM positive children was significantly higher than that in MP-IgM negtive children (P 〈0.01). Conclusion RTFQ-PCR diagnosis of MP infection in children has a favorite accuracy. The greater the fluorescent of copies are, the support with the diagnosis of MID infection goes up. It may have a higher diagnostic value in early onset and older children.
