摘要
目的分析RhD(-)红细胞(RBC)的预约受理和供应情况,评价RhD(-)RBC预约供应管理模式的有效性。方法回顾性分析本中心2006-2013年RhD(-)RBC的供应情况,用统计学方法分析改进RhD(-)RBC预约和供应方式前(2006-2009年)后(2010-2013年)RhD(-)RBC的供应量、调剂血量、接近失效期而发放给RhD(+)患者输注的血量,以及2012-2013年各临床科室RhD(-)RBC实际使用血量与预约血量的符合率。结果 1)改进预约与供应方式后RhD(-)RBC的供应量、调剂血量和接近失效期而发放给RhD(+)患者输注的血量均比改进前明显降低(P<0.01);2)2012-2013年RhD(-)RBC实际使用血量与预约血量符合率最高的是内科,其次是妇科和外科,最低的是产科。结论改进预约受理和针对不同临床科室采取不同供应策略后,RhD(-)RBC使用量降低,浪费减少,显示当前预约受理和供应管理模式既能满足临床用血需求又有效节约了稀有血型血液资源。
Objective To investigate the efficacy of a rare blood management system after an introduction of the new appointment - based order and supply of RhD negative red blood cell (RBC) in Fujian Blood Center. Methods The RhD negative RBC supply from 2006 to 2013 was retrospectively analyzed. The amount of RhD negative RBC supply, adjusted volume and transfusion by RhD positive patients near expired date were compared before (from 2006 to 2009) and after implementing the program (from 2010 to 2013). The coincidence rates of using and ordering RhD negative RBC of different clinical departments from 2012 to 2013 were also analyzed. Results Compared to the results collected from 2006 to 2009, all of the amount of RhD negative RBC supply, adjustment volume and transfusion by RhD positive patients near expired date from 2010 to 2014 decreased significantly (P 〈0. 01 ). The coincidence rates of using and ordering RhD negative RBC of the departments of internal medicine, gynecology, surgical and obstetrics were 85.5%, 53.5%, 41.3% and 6.5%, respectively. Conclusion The new appointment-based order and supply program of RhD negative RBC was proved to be a good practice to improve rare blood supply management, which optimizes RhD negative blood use and reduces waste.
