摘要
目的探讨经阴道彩超检测子宫动脉、子宫内膜及内膜下血流对体外受精-胚胎移植(in vitro fertilization-embryo transfer,IVF-ET)结局的影响。方法选取2010年6月至2012年6月安徽省妇幼保健院生殖中心行IVF-ET或卵胞浆内单精子注射(intracytoplasmic sperm injection,ICSI)治疗的165例患者的临床资料,根据妊娠结果分为妊娠组66例和未妊娠组99例。使用经阴道彩色多普勒超声成像技术监测注射人绒毛膜促性腺激素(human chorionic gonadotropin,HCG)日子宫内膜厚度及类型,ET日子宫动脉的收缩期与舒张末期流速比值(systolic/diastolic ratio,S/D)、阻力指数(resisitance index,RI)、搏动指数(pulsatility index,PI)、子宫内膜及内膜下血流等指标,比较两组间的各项指标。结果两组子宫内膜厚度及子宫内膜类型比较差异无统计学意义(P>0.05);妊娠组子宫内膜有血流所占比例[45.45%(30/66)]高于非妊娠组[25.25%(25/99)](P<0.05);妊娠组子宫内膜下血流RI(0.30±0.05)低于非妊娠组(0.69±0.07)(P<0.05)。结论经阴道彩超检测子宫内膜血流及子宫内膜下血流RI对IVF-ET结局有一定预测价值。
Objective To investigate the influence of uterine artery, endometrium and endometrial blood flow on the pregnancy outcomes of in vitro fertilization - embryo transfer ( IVF - ET). Methods 165 patients underwent IVF - ET cycles in Anhui Province Maternity and Child Health Hospital from Jun 2010 to Jun 2012 were recruited, who were divided into pregnancy group ( n = 66) and unpregnancy group ( n = 99 ) according to pregnany outcomes. Transvaginal color Doppler sonography ( TV - CDS) examination was performed to determine endometrial thickness, endometrial pattern on human chorionic gonadotropin (HCG) day, injection and systolic/diastolic ratio (S/D), resisitance index (RI), pulsatility index (PI), endometrium and endometrial blood flow on ET day, the indicators of two groups were compared. Results The differences of endometrial thickness and endometrial pattern between two groups had no significant difference (P 〉 0. 05) . The proportion of endometrial blood flow in pregnancy group [ 45.45 % (30/66) ] was significantly higher than that of unpregnancy group [ 25.25 % (25/99) ] ( P 〈 0. 05 ). The RI of endometrial blood flow in pregnancy group ( 0. 30 ± 0. 05 ) was lower than that of unpregnancy group (0. 69 ±0. 07 ) ( P 〈 0. 05 ). Conclusion Using endometrial blood flowand RI of subendometrial blood flow detected by TVS to predict the outcomes of IVF - ET is valuable.
