摘要
目的观察骨科手术中自体血液回输对红细胞渗透脆性的影响。方法 30名骨科大手术患者,检测术前、术中自体回收血和自体回输血及术后1d红细胞渗透脆性(OFmix、OFmax和溶血率)和红细胞平均体积(MCV)的变化。结果术中回收血液与回输红细胞OFmix(0.41±0.04)与(0.47±0.05),MCV(89.53±3.57)与(91.77±4.1)比较,溶血率(29.50±14.06)与(9.92±6.12)比较,均为P<0.01;患者术前和术后1 d血液比较,以上红细胞指标均统计学意义。结论骨科大手术中,术中回收血液经离心洗涤后,回输红细胞溶血率显著下降,术中自体血回输对患者红细胞渗透脆性未产生明显的影响。
Objective To investigate the influence of intraoperative blood salvage on erythrocyte osmotic fragility during orthopedic surgery. Methods Thirty patients who underwent orthopedic surgery were randomly selected. Blood samples were taken respectively. Erythrocyte osmotic fragility( including OFmix,OFmax and the ratio of hemolysis) and mean cell volume( MCV) were recorded before operation,before cell salvage in the reservoir,after blood processing in the transfusion bag,and one day after operation. Results Comparing the intraoperative cell salvage with blood processing in the transfusion bag,these following parameters OFmix( 0. 41 ± 0. 04) vs( 0. 47 ± 0. 05) and MCV( 89. 53 ± 3. 57) vs( 91. 77 ± 4. 1) showed a significant increase( P〈 0. 01). The hemolysis ratio( 29. 50 ± 14. 06) vs( 9. 92 ± 6. 12) significantly decreased after wash-ing in the transfusion bag( P〈 0. 01). There was no difference between erythrocyte osmotic fragility and MCV in the peripheral blood from the patient before operation and one day after operation. Conclusion During orthopedic surgery, intraoperative blood salvage has no adverse effect on erythrocyte osmotic fragility. The hemolysis ratio of RBC in the transfusion bag significantly decreased after washing.ly decreased after washing.
